<br />. <br /> <br />. <br /> <br />PROPOSED AMENDMENT NO.2 <br />TO <br />THE CITY OF LA PORTE EMPLOYEE MEDICAL FUND PLAN <br /> <br />Plan Document section SCHEDULE OF BENEFITS sub-section Maximum Plan <br />Payments is hereby amended to include the following: <br /> <br />Private Duty Nursing <br /> <br />$20,000 per lifetime <br /> <br />Plan Document section DEFINITIONS, C. PARTICIPATING DEFINITIONS, 1. Person <br />eligible for coverage and normal date of coverage: is hereby amended as <br />follows: <br /> <br />a. Covered Person is an Employee, 'Retiree, or Dependent who is an eli- <br />gible person enrolled in the Plan. <br /> <br />c. Retiree is a person who is a former employee covered under this Plan <br />on the date of retirement. <br /> <br />A former Covered Employee is eligible for coverage during retirement <br />if the following conditions are met at the date of retirement. <br /> <br />1. Is receiving retirement annuities as a qualified annuitant under <br />the Texas Municipal Retirement System or retired in accordance <br />with City of La Porte personnel policies, and <br /> <br />2. has completed at least three continuous years of service as a <br />full-time employee of the City of La Porte immediately prior to <br />retirement. <br /> <br />d. Dependent is the spouse or eligible child of a Covered Employee or <br />Retiree and who is enrolled in the Plan. <br /> <br />A child may be covered from birth up to the child's 19th birthday, <br />or up to the child's 25th birthday if such child is attending an <br />accredited school on a full time basis. Any dependent child less <br />than twenty-five (25) years of age who has graduated from an accred- <br />ited school and who has not become gainfully employed will be con- <br />sidered a dependent for a period not to exceed two (2) months. <br /> <br />The term "child" includes a natural child, adopted child, foster <br />child and step-child of a Covered Employee, and includes a natural <br />child, adopted child, or step-child of a Covered Retiree who was <br />covered continuously at least three years prior to the date of <br />retirement, or enrolls as a newborn. The child must be unmarried, <br />dependent upon the Employee for support and maintenance and reside <br />with the Employee or Retiree as legal guardian in a parent-child <br />relationship. <br />
