Credit card Account Changes <br /> <br />If changes are necessary to an existing credit card account, a Credit card <br />Account Change form should be completed, the appropriate approvals <br />obtained and a copy forwarded to the Program Administrator, The <br />Program Administrator will contact the Cardholder if any additional <br />information is needed. <br /> <br />I. Closing a Credit card Account <br /> <br />In the event t hat a n account must be closed, the Department Approving <br />Official should notify the Program Administrator and Authorizing <br />Official, and assist the Cardholder with the closing procedures detailed <br />below. If the account must be closed immediately, e-mail the Program <br />Administrator so they can verbally close the account with the Bank Card <br />Vendor and then follow the procedures described below. <br /> <br />Account Closing Procedures: <br /> <br />1. The Cardholder and the Approving Official must jointly complete the <br />Credit Card Account Change form. <br /> <br />2. Cut the credit card in half and send it with the above form to. the <br />Program Administrator, <br /> <br />10 <br />
