<br />. <br /> <br />e <br /> <br />ORDINANCE NO. 2002-2594-A <br /> <br />AN ORDINANCE AMENDING ORDINANCE NO. 2002-2594, nAN ORDINANCE <br />AUTHORIZING AND APPROVING A CONTRACT AMENDING THE MEDICAL PLAN <br />DESIGN, PROVIDED BY THE CITY OF LA PORTE AND ADMINISTERED BY THE <br />TEXAS MUNICIPAL LEAGUE INTERGOVERNMENTAL EMPLOYEES BENEFIT POOL <br />(TML-IEBP)...n, BY AMENDING THE CITY OF LA PORTE EMPLOYEE MEDICAL <br />FUND PLAN DOCUMENT RELIGIBILITY AND PARTICIPATIONn, KAICING VARIOUS <br />FINDINGS AND PROVISIONS RELATING TO THE SUBJECT, FINDING COMPLIANCE <br />WITH THE OPEN MEETINGS LAW, PROVIDING A SEVERABILITY CLAUSE, AND <br />PROVIDING AN EFFECTIVE DATE HEREOF. <br /> <br />BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF LA PORTE: <br /> <br />Section 1. The City Council of the City of La Porte hereby <br />amends the "ELIGIBILITY AND PARTICIPATION" section of the current <br /> <br />City of La Porte Employee Medical Fund Plan document, effective <br />September 1, 2004, as follows: <br /> <br />"ELIGIBILITY AND PARTICIPATION PG. 6 <br /> <br />Who is Eliqible? <br /> <br />You are eligible to participate in this plan after 30 <br />consecutive days of active employment if you are a full- <br />time employee with the City of La Porte and are regularly <br />scheduled to work at least 40 hours per week. You may <br />also participate in this plan if you are a retiree of the <br />City of La Porte receiving retirement annuities as a <br />qualified annuitant under the Texas Municipal Retirement <br />System, have retired in accordance with the City of La <br />Porte personnel policies, have competed at least ten (lO) <br />years of service with La Porte and are currently employed <br />by the City at the time of retirement, and do not work <br />for another employer after retiring from the City. (See <br />your Human Resources representative for any additional <br />requirements. ) <br /> <br />Your eligible dependents may also participate. Eligible <br />dependents include: your lawful spouse who is legally <br />married to and living with you; natural children; <br />grandchildren; stepchildren; or other children for whom <br />you have care, custody and control under court decree. <br />A dependent child must be unmarried and rely on you for <br />primary support and maintenance. Dependent children <br />remain eligible until age 25. <br /> <br />You may not participate in this plan as an employee and <br />as a dependent and your dependents may not participate in <br />this plan as a dependent of more than one employee. If <br />you and your spouse are both medical plan eligible <br />employees for the City of La Porte, you may elect to be <br />covered as a dependent under your spouse instead of as an <br />employee. You may also enroll as a dependent under your <br />
