<br />e <br /> <br />e <br /> <br />A. Statement that new owner and/or operator has no immediate intent to <br />change the facility's operations and processes; <br /> <br />B. Identification of the specific date on which the transfer is to occur; <br /> <br />C. Acknowledgement of full responsibility for complying with the existing <br />wastewater discharge permit. <br /> <br />Failure to provide advance notice of a transfer renders the wastewater discharge <br />permit invalid on the date of the facility transfer. <br /> <br />5.6 Wastewater Discharge Permit Revocation <br /> <br />Wastewater discharge permits may be revoked for the following reasons: <br /> <br />A. Failure to notify the Director of significant changes to the wastewater <br />prior to the changed discharge; <br /> <br />B. Failure to provide prior notification to the Director of changed conditions <br />pursuant to Section 6.5; <br /> <br />D. Misrepresentation of fully disclosing all relevant facts 10 wastewater <br />discharge permit; <br /> <br />E. Falsifying self monitoring reports; <br /> <br />F. Tampering with monitoring equipment; <br /> <br />G. Refusing to allow the director timely access to the facility premises and <br />records; <br /> <br />H. Failure to meet effluent limitations; <br /> <br />I. Failure to pay fines; <br /> <br />J. Failure to pay sewer charges; <br /> <br />K. Failure to meet compliance schedules; <br /> <br />1. Failure to complete a wastewater surveyor the wastewater discharge <br />permit application; <br /> <br />M. Failure to provide advance notice of the transfer or requirement, or any <br />terms of the wastewater discharge permit or the ordinance. <br /> <br />Wastewater discharge permits shall be void upon nonuse, cessation of operations, <br />or transfer of business ownership. All wastewater discharge permits are void <br />upon the issuance of a new wastewater discharge permit. <br /> <br />21 <br />
