<br />3. Regular full-time employees, hired prior to November 19, 1991, who <br />terminate shall receive, upon separation, payment for accrued but unused <br />sick leave, not to exceed 480 hours. Twenty-four hour shift personnel, <br />who terminate under the same conditions as above, shall be paid for <br />accrued but unused sick leave, not to exceed 768 hours. <br /> <br />4. Regular full-time employees hired after November 19, 1991, and who <br />terminate after 10 years of employment with the City, shall receive, upon <br />separation, payment for accrued but unused sick leave, not to exceed 480 <br />hours. Twenty-four hour shift personnel who terminate under the same <br />conditions as stated in this paragraph, shall be paid for accrued but <br />unused sick leave, not to exceed 768 hours. <br /> <br />5. Regular full-time employees hired after November 19, 1991, and who <br />terminate prior to 10 years of employment with the City, shall not be <br />eligible to receive payment for accrued but unused sick leave upon <br />separation. <br /> <br />Use <br /> <br />An employee may use sick leave for: <br /> <br />1. The employee's illness or injury that prevents the employee from working <br /> <br />2. To receive medical care from a physician, dentist, or other health care <br />provider <br /> <br />3. In the event of an illness within the employee's immediate family or <br />household, or for routine health care appointments for the employee's <br />immediate family or household that cannot be reasonably scheduled <br />outside of working hours (Limited to 3 full days) <br /> <br />4. Sick leave exceeding 14 days must be in coordination with FMLA <br />Leave. <br /> <br />Sick leave exceeding 5 workdays in any 6-month period may cause an employee <br />to be placed on physical report status. Employees under this status shall be <br />required to submit to their supervisor a medical doctor's statement of fitness for <br />duty upon returning to work from an illness. Failure to submit such medical <br />statement may result in disciplinary action. Physical report status shall remain in <br />effect for 6 months from the date of the action. <br /> <br />Sick leave shall not be used for unexcused absences. <br /> <br />Employee Policies Handbook/Page 26 <br />
