<br />City of La Porte - Main Street Program <br /> <br />P & Z, 2-19-09 <br /> <br />Enhancement Grant <br /> <br />MAIN STREET ENHANCEMENT GRANT <br />ApPLICATION - AGREEMENT FORM <br /> <br />Please return your completed application, along with required attachments, to the Main Street Development <br />office at 604 W. Fairmont Parkway, 2818-470-5017. A Main Street Design Certificate is required before the <br />Permit Division, 281-470-5074, is able to grant you a permit. <br /> <br />I have met with the Main Street Design Review Assistance Team and fully understand the Main Street <br />Enhancement Grant procedures and details established. I intend to use these grant funds for the <br />renovation projects, as spelled out in the MS EG and application. I have not received, nor will I receive <br />insurance monies for this revitalization project, or I have disclosed all pertinent insurance information <br />needed. <br /> <br />I understand that if I am awarded a MS EG by the MS DART, any deviation from the approved project <br />may result in the partial or total withdrawal of the MS EG.
