(GET!) is pleased to submit this lump sum cost proposal of: Four <br />Thousand. Six Hundred Dollars & No/100 ($4.600.00) to complete the demolition which includes: <br /> <br />1. Supervision, Labor, materials and equipment to accomplish the demolition including the cement slabs and <br />driveway to the ditch in accordance with the scope of work dated October 10, 2oo1provided by the City <br />of La Porte. <br />2. Back fill as necessary to prevent ponding. <br />3. Transportation and disposal of the demolition debris. <br /> <br />Notes and Clarifications: <br /> <br />1. All work shall be done co'htinuously working l()"hour days as necessary. <br />2. It is estimated the project will take two to three working days including backfill. <br />3. Proposal rates are based on GETI's current insurance package (attached). Should additional insurance be <br />required, additions shall be billed at cost plus Ten Percent (10%). <br />4. Changes in the original work scope initiated by the City of La Porte could result in cost adjustments. <br />Should this occur all changes and cost adjustments shall be agreed upon,by both parties p'rior to <br />implementation. <br />5. This proposal shall be valid for a period of 30 days. <br />6.
