If this Agreement is treminated due to the fault of City, Section II hereof <br />relative to termination shall apply. <br />13. Any notice permitted or required to be given to City by GCP~SCS hereunder may be <br />given by Certified United States Mail, Return Receipt Requested, addressed to PARKS ;;;JD <br />RECREATION, City of Laporte, P.O. Box 1115, Laporte, Texas 77571 and shall be considered <br />exercised and completed upon deposit in the United States f•1ai7. <br />14. The term of this Agreement shall begin on December 1, 1978, and continue in full <br />force and effect fora period of twelve (12) months to November 30, 1979, unless ter,:~i- <br />Hated earlier as provided. <br />15. City agrees to observe the schedule for service delivery as directed by GCMSCS. <br />Said schedule is attached hereto as Exhibit "B" and made a part hereof for all intents <br />and purposes. No holidays other than project holidays will be observed. <br />16. City agrees to cooperate and assist in any efforts undertaken by GCMSCS or the <br />• Governor's Committee on Aging to evaluate the effectiveness, feasibility, and costs of <br />the program. <br />17. City agrees to provide one representative and one alternate to be elected by <br />and from among program participants at the above mentioned site to serve on the Project <br />Council for the election of representatives and their alternates. <br />18. City agrees that no personal information obtained from an individual in con- <br />junction with the program shall be disclosed in a form in which it is identified with <br />him without written consent of the individuals concerned. <br />19. City agrees to keep records and make such reports in such form and containing <br />such information as may be required by GCMSCS and in accordance with guidelines issued <br />by the Administration on Aging. <br />20. City is cognizant of and agrees to operate the program fully in conformance <br />• with all applicable State and local standards, including fire, health, safety and sani- <br />tation standards, prescribed in law or regulations. Additionally, City covenants and <br />agrees that it will comply with any and all Federal statutes, laws and regulations re- <br />garding its obligations and duties undertaken herein and pursuant to the terms of this <br />Agreement and said grant award and application. <br />n <br />U <br />
