• <br />CITY OF LA PORTE <br />• INTER-OFFICE MEMORANDUM <br />T0: Jerry Hodge DATE: 9-15-80 <br />FROM: D.A. Paulissen <br />RE: Pipline Permit - P.L.T. Engineering <br />A review meeting was held 9-11-80 with the following people <br />in attendance: Jerry Hodge, Chacko Thomas, & D.A. Paulissen. <br />Below are the items that were discussed. <br />1. The Contractor must notify the City of La Porte 24 hours <br />prior to any crossing of any city utility with the pipeline. <br />The notice is required because the City must locate said <br />utility for the contractor. <br />2. The minimum ground cover requirements must be in accordance <br />with the City Pipleine Ordinance ~~915. In review of the profiles, <br />these requirements have been met. <br />3. All crossings of streets, street R.O.W. and improved R.O.W. <br />must be scheduled a minimum of 24 hours in advance with the <br />• City Public Works department. Note; all street crossings shall <br />be bored and cased and the bore must be inspected and approved <br />by the City Construction Inspector. <br />4. Any or all damages to city facilities, utility pipelines, <br />streets, etc. shall be reported immediately and repaired at no <br />cost to the City. Repairs must be inspected and approved by the <br />City Construction Inspector. <br />5. The pipeline contractor must place a 24 hour phone number on <br />file with both Public Works and the Inspection Division. <br />6. Streets must not be blocked without prior approval of the <br />Director of Public Works. <br />7. Copies of all pipeline testing shall be filed with the City <br />Inspection Division prior to the introduction of any product <br />into the said pipelines. <br />8. The City needs a start date of the pipeline entering La Porte. <br />Contractors will keep the City informed of all construction <br />progress while within the City limits. <br />• <br />
