A written report of <br />t'r.e hearing decision -hall be furnished to the holde~~ <br />of the permit,. license, or certificate by the Harris <br />County Health Department and kept on file at the City <br />of La Porte. <br />(f) Application after revocation - '~lhenever a revocation of <br />a permit has become final pur<uant to the determination <br />of the Director of the Harris County health Department, <br />the holder of the revoked hermit ~iav make written <br />application for a new permit to the Health department. <br />(g} Permit Fees - A health permit fee, au~ establishr~d by <br />the Commissioners Cot:rt in unicorporated areas of the <br />county, is required annually for each food <br />establishment. F?es are due and payable on or before <br />one (1} year frorn date of issuance and are to be paid <br />to the Harris County Health Department at 2501_ Dunstan, <br />Houston, Texas. <br />Section 11-7. Review of Plans. <br />(a) Submission nf_ Plans - Whenever a food establishment is <br />constructed or extensively remodeled and whenever an <br />existing strricture is converted to use as a food <br />establishment, property prepared plans and <br />specifications for such construction, remodeling, or <br />conversion shall be submitted to the City of La Porte <br />and the Harris County Health Department for review and <br />approval before construction, remodeling or conversion <br />is begun. The plans and specifications shall indicate <br />the proposed layout, arrangement, mechanical plans, and <br />construction materials of work areas, and the type and <br />model of proposed fixed equipment and facilities. The <br />City of La Porte and the Harri:~ County Health <br />Department shall approve the plans and specifications <br />if they meet the requirements of these rules and the <br />regttirernents for the issuance of a Building Permit from <br />anyone from the normal procedures for obtaining <br />Building Permits frorn the City of La Porte. No food <br />establishment shall be constructed, extensively <br />remodeled, or converted except in accordance with plans <br />and specifications approved by the Harris County Health <br />Department and in accordance with the City of La Porte. <br />(b) Pre-operational Inspection - The Harris County Health <br />Department shall inspect any food establishment prior <br />to its beginning operating to determine compliance with <br />approved plans and specifications and with the <br />requirements of these rules. <br />Section 11-8. Food Manager Certification. <br />(a) Requirement - Every permitted food establishrent except <br />establishments dealing oril_v in: fresh produce: <br />nonpotentially hazardous food; and/or prepackaged <br />potentially hazardous food1 shall have a person <br />employed in ~~ managerial capacity processing a r_urrent <br />Page -4- <br />
