Limitation on Use of CDBG-Funded Facilities <br /> Facilities constructed with CDBG funds must be used for their intended purposes as described <br /> in the proposal for a period of time to be specified in a contract with Harris County. If the <br /> facility is proposed to be used for other purposes prior to the end of the specified period, <br /> approval must first be obtained from HCCDA. <br /> IV. PROPOSAL SUBMIIIAL AND REVIEW PROCESS <br /> A ProDosal Submittal <br /> Enclosed are one, several, or all of the following proposal forms: <br /> 1) Physical Improvements and Facilities; <br /> 2) Social Service Facilities; <br /> 3) Social Service Programs; and <br /> 4) Housing Programs. <br /> Complete and submit the first page of the appropriate proposal form, and answer the <br /> questions on the remaining pages, using your own paper. Include all elements contained in the <br /> proposal form. Proposals must be complete and self-explanatory and not require oral <br /> explanation. Non-profit agencies should attach Exhibit 1, Proposer Certification; Exlnbit 2, <br /> Authorized Signatures; and Exhibit 3, Accounting System. <br /> Answer all questions in a clear, concise manner. The Certifying Representative listed on page <br /> one of the proposal should be the individual who has legal authority to submit proposals for <br /> the agency or jurisdiction and enter into agreements 'with Harris County, i.e., the Chairman of <br /> the Board of a social service agency or the Mayor or City Manager of a co-op city. The <br /> Certifying Representative may be different from the contact person listed on that page. <br /> SUBMIT ONE ORIGINAL AND THREE COPIES OF THE PROPOSAL. (please Note: <br /> Non-profit agencies need submit only one copy of Exhibit 1) All proposals are due in the <br /> office of the Harris County Community Development Agency at 3100 Timmons Lane, Suite <br /> 220, Houston, Texas 77027 by 3 D.m. on Fridav. December 18. 1992, There will be a penalty <br /> for late or incomplete proposals. <br /> B. ProDosal Review <br /> Proposals will be reviewed by a 13 member committee comprised of HCCDA staff who may <br /> request additional information, meetings and/or site visits with applicants. The committee will <br /> evaluate and rank the proposals and submit the rankings to the HCCDA Director for funding <br /> recommendations (evaluation criteria will be available to applicants upon request). A list of <br /> projects recommended for funding will be published in late April, 1993. The Housing <br /> Authority Board of Commissioners will review the funding recommendations in early May, <br /> 1993 and will forward its recommendation to the Commissioners Court for consideration in <br /> mid May. Applicants will be notified in writing regarding the status of their proposals after <br /> CommisSioners Court action. <br />
