<br />City of laporte.Public Works <br />PSI Proposal No.: 0204-12683.Rev-1 <br />1/20/2010 <br />Page 2 of 9 <br /> <br />PSI proposes to provide experienced qualified technical personnel to perform services <br />in general accordance with the project plans and specifications. PSI will perform these <br />services as requested by the client. Michael J. Whitecotton, PE will be the responsible <br />registered Engineer for your project. <br /> <br />Scope of Services: <br /> <br />PSI understands the project will consist of the installation of approximately 13,000 linear <br />feet of sanitary force main, storm sewer, and waterlines along Sens Road from North 0 <br />Street to North L Street, along North H Street and along North L Street. There will also <br />be underground auguring of the utilities beneath pavements. We understand the <br />concrete and asphalt on the project will be temporary and will not require inspections or <br />testing. The project will require approximately 210 days for substantial completion. We <br />anticipate approximately 80 days of periodic inspections and testing for our services. <br /> <br />Based on this information, the plans, City of LaPorte Specifications, Harris County <br />Specifications, and our experience with similar projects, PSI proposes the following <br />scope of services for your project. <br /> <br />1. Earthwork - The earthwork scope includes the inspections and testing of the <br />installation of the utilities ranging from 4" up to 36" PVC, as well as the Manhole <br />and Fire Hydrant installations. We anticipate PSI's testing services to be on a <br />relatively full-time basis during the utility line installation portion. PSI will perform <br />the following field inspections and testing for the items described above, as per <br />the project specifications. <br /> <br />a) Sample subgrade, select fill, trench backfill, and cement stabilized sand. <br />Prepare and test the samples for classification by the Atterberg Limits, <br />and moisture density relationship. <br /> <br />b) Perform field density tests using the nuclear method to determine the <br />moisture content and percent compaction of the subgrade, fill, and <br />backfill. <br /> <br />c) Project Manaqement- Project Management for the project will consist of the <br />attendance of progress meeting if requested, review of concrete mix designs if <br />requested, the oversight of the testing, review and issuing of the reports of the test <br />results. <br /> <br />1115/1 <br />
