You should provide the applicant with written notice of the time and location of the meeting. <br />The applicant is not required to attend the meeting. <br /> <br />The local board should formally vote to approve or deny the award of benefits, and the action reflected in <br />the minutes of the meeting. If the local board approves the award of benefits, two board officers with <br />signature authority should sign and date the Service. Retirement Certification awarding benefits. Provide <br />copies of the signed and dated Service Retirement Certification to the applicant and to the department. <br />Mail the signed and dated original Service Retirement Certification to FFPC immediately upon adoption. <br /> <br />By copy of this letter, the applicant is directed to contact you in writing regarding any errors or <br />omissions in the FFPC Service Retirement Certification Qualified Service History. Corrections to the <br />applicant's enrollment or qualified service history must be completed before the local board approves the <br />applicant's retirement award. Do not approve this Service Retirement Certification if it contains incorrect <br />enrollment or qualified service history information from the applicant. <br /> <br />A person aggrieved by a decision of a local board relating to eligibility for or the amount of benefits may <br />appeal the decision to the Commissioner in accordance with the provisions of Government Code <br />9864.016. Written notice must be filed with the local board not later than the 20th day after the date of <br />the decision and a copy provided to the Commissioner. <br /> <br />Monthly service retirement annuity payments will commence after the approval of the local pension <br />board and the Commissioner. If your local board does not determine a claim for service retirement <br />benefits and file its determination with the Commissioner before the 16th day after the date the local <br />board receives the claim, the Commissioner may determine the merits of the claim. <br /> <br />The applicant will receive payment from the retirement date identified above. The cashing or depositing <br />of the first check is considered acceptance of the amount of the annuity and is conclusive evidence that <br />the payee is retired. <br /> <br />Thank you for your attention to this matter.
