City of La Porte <br />Emergency Medical Services <br />10428 Spencer Highway <br />La Porte, Texas 77571 <br />(281) 471-9244 <br /> <br /> Ray Nolen <br /> Chief <br /> <br /> <br /> <br /> <br /> <br /> <br />September 25, 2017 <br /> <br /> <br />Emergency Medical Services Provider Agreement <br /> <br /> <br />The current 3-year agreement between the City of La Porte and facilities located within the Battleground Industrial <br />District for Emergency Medical Services will expire on September 30, 2017. <br /> <br />The City of La Porte would like to re-new these particular agreements during the upcoming months to facilitate <br />continuous coverage of Emergency Medical Service for your facility. The current agreement was based on a fee of <br />$27.43 per on-site employee. The attached 3-year agreement renewal reflects a slight increase to $29.37 per on-site <br />employee due to needed increases implemented within the operational budget of the City of La Porte EMS. <br /> <br />Facilities located within the Battleground Industrial District that elect to not enter into an agreement and remit <br />associated fees will continue to receive Emergency Medical Services responses provided by the City of La Porte as <br />-000.00 will be charged to the Non-Member facility on a per response <br />basis.
