The Sample Company uses a participatory strategy rewarding employees for healthy behaviors. <br />Employees can earn up to $100 for participation in the wellness opportunities. Although the <br />Sample Company has developed a platform to encourage, develop, and maintain healthy <br />lifestyles, participation in the programs has been minimal (less that 13% of employees <br />participated in Biometrics in 2013). <br />Short Term Strategies: <br /> Maintain a participatory wellness program. To increase engagement, revise the incentive <br />structure to include a health insurance premium reduction between $25 and $50 per <br />paycheck for employee completion of Health Assessment, Biometric Screening and Health <br />Coaching is recommended. <br /> Establish a health insurance premium reduction for non-Tobacco Users. A per paycheck <br />reduction between $25 and $50 for employees, retirees and spouses is recommended. <br /> Include spouses in Wellness Program and rate differential / incentive structures starting <br />in 2016 for premium reduction in 2017. <br /> Re-launch the Sample CompanyP <br />MotivateMe incentive platform and Zensey digital engagement platform. <br /> Implement and incentivize use of Cigna Healthy Baby program to target expectant <br />mothers and high risk pregnancies. <br /> Establish plan participation and performance criteria to measure the short and long term <br />success of the Wellness Program. Example: <br />- Annual Participation: Minimum of 50% of population participates in one core <br />program annually. <br />- Biometric Participation: 80% of population participates in Biometric Screening <br />and Health Risk Assessment. <br />- Health Risk Reduction: Annual health risk shift of 2+% is defined as a decrease in <br />the high-risk cohort or those with Metabolic Syndrome. Baseline biometric <br />measurement in 2015 and compare to biometric measurement in 2016 (Cigna <br />will provide reporting if screenings are provided by Summit Health). <br />- Return on Investment (claims or risk factor models): 0.5:1 at the end of first year <br />of full program implementation. 2:1 at the end of the second year. <br /> Re-evaluate disease management strategies and participation incentives for programs. <br />Long Term Strategies: <br /> Consider moving from a participatory approach to wellness to a results based approach, <br />providing incentives to participants who meet pre-established biometric criteria centered <br />on risk for Metabolic Syndrome. <br /> Consider on-site or near-site physician services to enhance primary and preventive care <br />utilization. <br /> Evaluate targeted coaching and educational programs for employees with high risk health <br />conditions (ex.
