<br />Exhibit A <br /> <br />SCOPE OF SERVICES <br />I. Application <br />This Scope of Services is based on the proposal prepared and submitted by La Porte. However, in the <br />event of any conflict between the proposal and any provision contained herein, this Agreement shall <br />control. In addition to the activities listed below, La Porte agrees to operate this Project in accordance <br />with the FTA 5307 Grant Program requirements and all other applicable Federal, state and local <br />regulations. <br /> <br />II.Project Description <br /> <br />The scope of the Project is to install up to 4,400 linear feet of sidewalk and 23 ADA ramps along Park <br />Drive within the City of La Porte as shown in Exhibit G. The sidewalks being <br />replaced are dilapidated with numerous tripping hazards. The new sidewalks will significantly reduce <br />traffic danger to pedestrians along these routes. This project will benefit users of the fixed route transit <br />system and enhance mobility access for individuals who reside or work within La Porte. <br /> <br />La Porte shall be responsible for implementing the Sidewalk Improvements Project during the term of <br />this Agreement as follows: <br /> <br />PROJECT LOCATION SIDEWALK # OF ADA RAMPS <br />(LINEAR FT) <br />4,400 23 <br />Park Drive <br />from S. Lobit Ave to San Jacinto Street <br />TOTAL 4,400 23 <br /> <br />La Porte shall administer all activities in the provision of the aforementioned sidewalk improvements <br />project in accordance with all applicable Federal, state, and local rules and regulations governing these <br />funds, and in a manner satisfactory to the County. <br /> <br />La Porte shall ensure that personnel providing services under this Agreement have all licenses required <br />by law and/or are qualified to perform the services required under this Agreement. La Porte shall further <br />ensure that all Program and/or facility licenses necessary to provide the required services are current and <br />that the County shall immediately be notified if any such required licenses become invalid or are <br />canceled during the term of this Agreement. <br /> <br />III. Activities <br /> <br />A. Operations and Maintenance <br />La Porte shall be responsible for the operations and maintenance of the sidewalk improvements <br />described in Exhibit A upon completion of construction. <br /> <br />B.
