(CLEMC). However, CLEMC is closing on <br />November 15, 2019, and will no longer be providing services. The Port of Houston Authority <br />(PoHA) reached out to the City of La Porte to request emergency medical services to the <br />Bayport Terminal beginning November 1. <br />At this time, PoHA contracts with the City of La Porte for EMS services at the Barbour's Cut <br />Facility for the period of February 1, 2018, until the expiration on January 31, 2021. This service <br />contract has worked well for the provision of emergency services in the past and PoHA would <br />like to add this service to their Bayport Terminal. The Bayport Terminal will not have emergency <br />medical services effective November 1, 2019, and requests to contract for these services with <br />the City of La Porte EMS for a period of fifteen (15) months to coincide with the current contract <br />for Barbours Cut. <br />La Porte EMS has provided the following responses to the Port of Houston — Barbour's Cut <br />Terminal: <br />FY2016-17 — <br />13 responses out of 3,888 total calls <br />FY2017-18 — <br />15 responses out of 4,046 total calls <br />FY2018-19 — <br />16 responses out of 4,080 total calls <br />The attached 2019-2021 contract is calculated at the same rate as the City's existing contract <br />with the Port of Houston Barbour's Cut Terminal, at the rate of $30.52 per employee per year. <br />The total agreement amount invoiced will be based on an Average Daily Population (ADP) of <br />645 employees at the Bayport Terminal. <br />The Port of Houston Authority — Bayport Terminal agrees to pay fifteen (15) monthly payments <br />of $1,640.45. The total amount of the contract for fifteen (15) months is $24,606.75. Contract <br />fees are in addition to any and all patient billing and revenues for actual calls for service. <br />
