(d) $10.00 charge, if any, for unfounded requests for service. - (e) $15.00 maximum charge for transporting ' s office one person to a di~ctor in La Porte, from location within the limits of ~hc City of La Porte. (f) A7ethod of dividing cost ~~rrong patients if more than imultaneously one is carried in the same arnhulance s to one hospital: One an d one-half .times the regular rate, divided by the number of patients transported; each patien t to pay equal amount. The collection of such fees shall be the sole responsibility of tl~e Contractor. vI . CONTRACTOR REQUIRED TO FURNISH ~~'RI'I"I'EN REPORTS At tine c~:piration of the first three »onths' period and every six months thereafter, Contractor shall file with the City Clerk during the month immediately following each such period a composite, cumulative report on the emerycncy ambulance service, both within City and within the boundaries of the La Porte Independent School District, showing, to-wit:. (a) Nwnber of calls made; (b) Number of calls for service which have been paid for and amount received by .Contractor for such service; (c) Number of calls not paid for and amount of such unpaid charges; and (d) Proof of response, from tirne.of call to arrival at scene.
