• • ORDINANCE NO. ~~~~ t/ AN ORDINANCE ADOPTING AMENDMENT NO. 2 TO THE CITY OF LA PORTS EMPLOYEE MEDICAL FUND PLAN; AND PROVIDING AN EFFECTIVE DATE HEREOF. BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF LA PORTS: Section 1. Amendment No. 2 to the City of La Porte Employee Medical Fund Plan, in form attached hereto as "Exhihit ~," is hereby adopted and approved by the City Council of_ the City of La Porte. Section 2. Retirees and their Dependents who elect to participate in the Plan shall pay the full amount of the costs of their coverage, as determined from time to time by the Plan's actuaries. The City of La Porte shall make no Contribution toward the cost of coverage of Retirees and their Dependents. Dependents shall include Dependents of any deceased Employee or Retiree, as defined in Exhibit "A", attached hereto. Section 3. Current eligible Retirees and their Dependents shall have a thirty-day enrollment period, following certified mail notification to them by the administrator, of the availab- ility of Coverage. Future Retirees and their Dependents must elect coverage prior to their last day of employment. Section 4. The City Council officially .finds, determines, recites and declares that a sufficient written notice of the date, hour, place and subject of this meeting of the City Council was posted at a place convenient to the public at the City Hall of the City for the time required by law preceding this meeting, as required by the Open Meetings Law, Article 6252-17, Texas Revised Civil Statutes Annotated; and that this meeting has been open to the public as required by law at all times during which this ordinance and the subject matter thereof_ has been discussed, considered and formally acted upon. The City Council further rati.f ies, approves and confirms such written notice and the contents and posting thereof. ~~
