<br />e <br /> <br />e <br /> <br />Page Three <br />La Porte EHS <br /> <br />Dr. Suarez moved to Houston 15 years ago to prac t ice a s a <br />Cancer Surgeon at M.D. Anderson Hospital. For the past 10 <br />yea rs, Dr. Sua rez ha s served a s an erne rgenc y physic ian in the <br />Bay town area. Eight of those ten years includes experience as <br />a Medical Director for various EMS Services. Dr. Suarez is a <br />member of the Harris County Medical Society, the American <br />College of Emergency Physicians, as well as an instructor of <br />Advanced Cardiac Life Support. <br /> <br />IV. PROPOSAL <br /> <br />The City of La Porte proposes to provide its Emergency Medical <br />Services (EMS) to the Bayport Industrial Association. EMS <br />services included in this proposal are as follows: <br /> <br />o two continually staffed MICU's; <br /> <br />o a reserve back-up ambulance; <br /> <br />o 911 Emergency Network availability; <br /> <br />o maximum response time of ten minutes; <br /> <br />o decontaminat ion and transportat ion of exposed plant <br />workers; and <br /> <br />o mutual aid agreements with Deer Park and Bay town, in the <br />event of calamity or disasterous conditions. <br /> <br />A copy of our existing industrial contract is enclosed in this <br />report. <br /> <br />V. RAt.1iS <br /> <br />The EMS rates submitted for consideration are as follows: <br />I <br /> <br />o five dollars per plant employees including all other <br />on-site personnel (i.e. contractors); and <br /> <br />0* Transports: $100 for up to 15 mile transport and $125 for <br />all transports beyond 15 miles. <br /> <br />* (Note: These rates may increase, pending outcome of <br />proposed ,transport rate increase. If approved, the new <br />rates will be: $175 for up to a 15 mile transport; and <br />$200 for all transports beyond 15 miles) <br />
