Random testing will be conducted at a frequency <br />determined by the City with no advance notice as to the date <br />of testing, or the individuals selected for testing. At a <br />minimum, random testing will be conducted once per month <br />with at least 4 percent of the workforce tested each time. <br /> <br />3. The contract vendor that provides collection services <br />will perform the random selection process in a manner that <br />rules out any discretionary action by the City. The <br />identification of individuals for random testing will be <br />made on the basis of identifying numbers, such as social <br />security numbers. The selection process will not include <br />individual names. <br /> <br />4. An individual selected for random testing will be <br />instructed to report to the specimen collection site without <br />delay. <br /> <br />5. Failure to appear promptly at the collection site will <br />subject the individual to discipline. An exception to <br />discipline will be granted only when the individual presents <br />a valid reason in writing that is supported by a written <br />report of the individual's supervisor. <br /> <br />VII. Collection and Analysis <br />A. Specimen Collection <br /> <br />1. Any collection site designated for use in the City's <br />drug testing program shall have all necessary personnel, <br />materials, equipment, facilities, and supervision to provide <br />for the collection, security, temporary storage, and <br />shipping or transportation of specimens to a certified drug <br />testing laboratory. <br /> <br />2. Procedures shall provide for the designated collection <br />site to be secure. If a collection site facility is <br />dedicated solely to urine collection, it shall be secure at <br />all times, If a facility cannot be dedicated solely to drug <br />testing, the portion of the facility used for testing shall <br />be secured during drug testing. <br /> <br />3. Chain of custody standardized forms shall be properly <br />executed by the authorized collection site personnel upon <br />receipt of specimens. Handling and transportation of urine <br />specimens from one authorized individual or place to another <br />shall always be accomplished through chain of custody <br />procedures. Every effort shall be made to minimize the <br />number of persons handling specimens. <br /> <br />4. No unauthorized personnel shall be permitted in any part <br />of the designated collection site when urine specimens are <br />collected or stored. <br />
