<br />e <br /> <br />e <br /> <br />G. Laboratory Facilities <br /> <br />1. Laboratory facilities shall comply with applicable <br />provisions of any state licensure requirements and shall <br />have the capability, at the same laboratory premises, of <br />performing initial and confirmatory tests. <br /> <br />VIII. Recordkeeping <br /> <br />A. The contract vendor will maintain records of test <br />results on behalf of the City and will ensure that all <br />records related to the administration and results of the <br />drug testing program are maintained for a minimum period of <br />5 years except that individual negative test results will be <br />maintained for a minimum of 12 months. <br /> <br />B. The contract vendor will retain information that will <br />indicate only the following: <br /> <br />- That the individual submitted to a testing. <br /> <br />- The date and location of such testing. <br /> <br />- The identity of the drug testing laboratory which <br />performed the testing. <br /> <br />- Whether the test findings were "positive" or <br />"negative." <br /> <br />C. The contract vendor will produce upon demand and will <br />permit the City to examine all records related to the <br />administration and results of testing performed. <br /> <br />D. The contract vendor will maintain on behalf of the City <br />an annual (calendar year) summary of the records related to <br />the administration and results of the testing program. This <br />summary will include, at a minimum: <br /> <br />- The total number of tests administered; <br /> <br />- The number of tests administered in each category <br />(i.e., pre-emploYment, reasonable cause, post-accident, <br />random, return to work, and rehabilitation); <br /> <br />test; <br /> <br />- The total number of individuals who did not pass a <br /> <br />- The total number of individuals who did not pass a <br />test by testing category; <br /> <br />- The disposition of each individual who did not pass a <br /> <br />test; <br />
