<br />e <br /> <br />e <br /> <br />TEST CONSENT IN RETURN TO DUTY SITUATION; <br /> <br />City policy provides that an employee suspended for having <br />in his or her body system an unexplained presence of a <br />prohibited substance, such as drugs or alcohol, may be <br />required as a condition of reinstatement to enter the City- <br />approved rehabilitation maintenance program. <br /> <br />An element of the program may require the suspended employee <br />to undergo, cooperate with, and successfully complete an <br />approved course of therapy as a condition of reinstatement. <br />Further, if reinstated, the employee may be required to <br />remain enrolled in the program for the purpose of receiving <br />followup, reinforcing, or maintenance therapy. <br /> <br />If you are reinstated, you will be expected to remain free <br />of those substances prohibited by the City's drug abuse <br />policy, and you may from time to time be asked on an <br />unannounced basis to provide specimens of your urine and/or <br />blood plasma for laboratory analysis. If you refuse to <br />provide such specimens when requested to do so or if such <br />specimens reveal an unexplained presence of the prohibited <br />substances, you may be subject to disciplinary action, <br />including discharge or disbarment from the workplace. <br /> <br />RESPONSE: <br /> <br />I have read and understand the prov1s10ns regarding <br />reinstatement following a violation of the City's policy on <br />drug abuse. I agree to comply with those provisions. <br /> <br />I specifically consent at this time to give a specimen of my <br />urine and/or blood plasma for drug testing purposes. I also <br />give consent to the City and its agents, including the <br />collecting and testing agencies, to disclose and discuss the <br />results of such testing.
