Meeting Minutes of the City of La Porte <br />Chapter 172 Employee Retiree Insurance and Benefits Board <br />Meeting held on September 3, 2015 <br />1. Call to Order <br />The meeting was called to order at 3:02 p.m. by Traci Leach. <br />2. Member Roll Call <br />Present: Corby Alexander, Traci Leach, Clark Askins, Michael Dolby, Matt Daeumer, Aaron <br />Corral,es, Juliane Graham, Charles Montgomery and Lindsey Campuzano <br />Absent: Martha Gillett and Robert Eldridge <br />3. Consider approval of minutes from October 1, 2014 meeting <br />Matt Daeumer made a motion to approve the minutes as presented. Juliane Graham <br />seconded. Motion passed. <br />4. Discuss options for potential plan design changes for FY2,016 and beyond - McGriff, <br />Seibels & Williams of Texas, Inc. <br />Charles Montgomery made a motion to freeze the PPO 500 plan for New Hires, effective <br />January 1, 2016. Employees hired prior to Jan. 1, 2016 will have to option to move in and out <br />of the PPG 500 plan. Matt Daeumer seconded. All in favor, with the exception of Aaron <br />Corrales. <br />Discussion was held to propose an increase to the medical plan for tobacco users. No motion <br />was made. Board members will meet again to discuss an implementation process. <br />Charles Montgomery made a motion to add a specialty tier for prescriptions. New tier would <br />be an additional $,20 or 20%, depending on prescription, to a $100 maximum. Mr. Montgomery <br />also made a motion to implement DAW 2. DAW 2 occurs when a member requests a brand, <br />when generic is available, The Member will pay the difference between the brand and generic <br />plus the brand copay. Mr. Montgomery made a third motion for members to utilize a CVS <br />pharmacy to receive a 90-day supply of maintenance drugs with mail order copays applying. <br />Scripts may require a pre -authorization annually. Juliane Graham seconded all three motions. <br />Motions passed. <br />There will be a 2.35% increase to Aetna's Medicare Advantage Plan for Post 65 retirees in <br />2016, Increase per member will be $6.16. No motion needed. <br />5. Review proposals for Long -Term Disability, Short -Term Disability, Dental and Vision <br />plans and provide recommendation for Council — McGriff, Seibels & Williams of Texas, <br />Inc. <br />Matt Daeumer made a motion to change dental providers to CIGNA. Charles Montgomery <br />seconded. Motion passed. <br />Juliane Graham made a motion to change Long -Term and Short -Term disability provider to <br />VOYA. Charles Montgomery seconded. Motion passed. <br />
