HTE Support Services The: initial lelm of HTE application support services shall commence OD~ hundred twenty (120) days aftcr delivery of the Licensed Ptogram(s). and extend for a twelve (12) month term. Subsequent terms ofsuppon will be for twelve (12) monrn periods, comme:nci1lg ar me end of me initial support period. . Application Training Listed above, if applicable, arc the number of days of training for the Licensed Programs listed. Additional application training, if requested by the Customer, can be provided upon request at the standard billing 'fatl! in effect at that time, which :11 the time of this Suppltment is One Thousand Two Hundred Eighry DoJJars ($1,280.00) per day. The fee quoted does Qot include \ravel and living expenses. Project Managemeat A description of me applicable level of Project ManagemeDt is attached to this Supplement and fees will be invoiced in the amoun~ and under Ihe terms aoted above. A mu~ally agreeable work plan will be: ereate:d by the HTE Project Manager and lhe Customer. Additional days of Projcel Management, if requested by Customer. will be invoiced as incuned at the then current HTE daily rale at the standard billing rate in effect at that lime, which at the time of this Supplement is ODe Thousand Two Hundred Eighry Dollars (51.280.00) per day. The fee quoted does not include lr3vel and living expenses. Modifications Modifications will be controlled by the HTE sea form which will be prepared for the Customer by the RTE Product Manager responsible for that module. There will be a two hundred fifty dollar ($250,00) non-refundable processing fee for preparation of each SCR requested by the Customer. Conversion Convcrsion. if provided for herein, or if rcqueSlcd by the Customer after contract execution, wj]J be controlled by the HTE SCR Conn which will be prepared for the Custumer by the HTE Conversion Team Manager. There will be 41 two hWlCired fifty dollar (S250.00) non-rcfuudable processing fee for preparation of each SCR requested by the: Customer. HTE will proceed on the SCR when the signed SCR is retun,ed with the CUstomer's autboriz:UiOD aloD~ with fifty perceat (50%) payment, less lhe processing fee. The flDal fifty percent (50%) payment is due upon completioQ. Data must be given to HTE in an IBM compatible format Oll a specified magnetic media and must match data field defmitioD. Input data file cJean up shall be me responsibility of the customer. Additional conversion, if necessary, will be invoiced at the prevailing rate per hour. Cancenati~D Charges Customer acknowledges mat HTE makes evcry eiTort to schedule niniDg and project management sessions sufficiently in advance 10 make effective use of HTE.s personnel and to obtain favorable prices for lravel aod living. Accordingly, the following cancellation charges apply 10 training or on-site project management sessions c8I1celed at th~ request of Customer: cancellation wilhil1 seven (7) days of stan darc, Customer pays f1fly percent (SO"ID) of the totaJ price for the traiDiag or oo-site project management; C:lJlcellation within three (3) days of start date, Customer is responsible for entire price of the training or on-site project mauagemenl. In addition to the foregoing, Cuslomer shall be obligated 10 reimburse HTE for any nOD- re:fw\lJable expenses mculTed by HTE for lravc:1 expenses. Notwithstanding the above, HTE will endeavor CO reschedule HTE personnel in order to mitigate Customer's costs and expenses under this paragraph. To the extent HTE is successful in such rescheduling, Custome:r's payment obligations shall be reduced. Third Party Software and Hardware Unless otherwise stated, CUstomer shall enter into separat" licCDse and/or maintenance agreements widt eacb individual third party h3J'dwarc or third party software supplier for the equipment or software as may be provided herein.
