Staff reviewed the revised plat and found the following items need to be addressed.

1. Add address — 204 Sylvan Ave. (Res. A) & 206 Sylvan Ave. (Res. B).
2. Show zoning as Low -Density Residential (R-1).
3. Correct to show current owner's name in the acknowledgement execution statement on the plat.
4. Indicate name and locations of adjacent properties with legal description, acreage tracts or
HCAD information.
5. Add paragraph to the Owner's Acknowledgement and Final Survey Certificate on the plat (see

6. Show a survey control monument along street centerline or subdivision comer accurately tied
by angle and distance.

corporation), owner (or owners) hereinafter referred to as Owners (whether one or more) of the (number of acres) tract described in
the above and foregoing map of (Name of subdivision) do hereby make and establish such subdivision of said property according to
all lines, dedications, restrictions and notations on said maps or plat and hereby dedicate to the use of the public forever, all streets
(except those streets designated as private streets), alleys, parks, water courses, drains, easements and public places shown thereon for
the purposes and considerations therein expressed; and do hereby bind myself (or ourselves), my (or our) heirs, successors and assigns
to warrant and forever defend the title of the land so dedicated.
FURTHER, Owners have dedicated and by these presents do dedicate to the use of the public. for public utility purposes forever an
unobstructed aerial easement five (5) feet in width from a plane twenty (20) feet above the ground level upward, located adjacent to all
