Tosco 7725"U9 May 09 03 04:22p GRAY EHVIROMMEMTAL 291 333 2352 p.3 ASBESTOS SURVEY 100 West Main LaPorte, Texas +?si1IT11 On May 8, 2003, Mr. Greg Holle initiated an asbestos survey at 100 Rest Main located in LaPorte, Texas to determine the presence of Asbestos Containing Materials (ACM), if any. The survey was conducted by Gray Environmental Terbnology, Inc, which is a Texas licensed Consultant Agency, number 10-M2. Bulk samples were collected by Coral Slade, a Texas licensed Inspector, number 602261. DUSCUSSION The survey consisted of collecting bulk samples of building materials in the area to be disturbed during the renovation of the existing building. Bulk samples collected consisted of wall texture and joint compound in the locations sbowm on the attached sketch The existing wall where the samples were taken will be removed The attached table of results and laboratory report sew that a total of three samples were collected and were negative (did not contain) asbestos. ANALYTICAL SERVICE Analytical services were conducted by an accredited American Industrial Hygiene Association (AIHA), National Voluntary Laboratory Accreditation Program (NVLAP), and Texas licensed laboratory number 30-0176. CONCLUSION Based on the attached laboratory analytical report and Table of Results no Asbestos Containing Material (ACM) was detected. The required type of construction for the building is determined by applying the height and area limitations for each of the applicable occupancy groups to the entire building with the most restrictive type of construction requirements being applied, and 2. The entire building conforms with the most restrictive occupancy group fire protection requirements, as determined by Chapter 7 and 8, and 3. The entire building conforms with the most restrictive occupancy group sprinkler, standpipe and alarm system requirements, as determined by Chapter 9, and 4.
