Glazing Method Description Key:
GM-1: The insulated glass units are exterior glazed and are set against double -sided adhesive tape.
The insulating glass units are secured in place with rigid vinyl glazing beads.
Frame Construction: The frame members are constructed of extruded polyvinyl chloride (PVC). The
frame corners are mitered and welded construction. A snap -in rigid PVC adapter is located at the head.
Sash Construction: The sash members are constructed of extruded PVC. The sash corners are mitered
and welded construction.
Reinforcement:
System 1: Custom shaped aluminum reinforcement is utilized in the lock rail. All reinforcement
extends the length of the members.
System 2: None.
Hardware:
Metal cam -type lock and keeper; Two (2) required; Located 7 y inches from each end of the lock
rail. Engages metal keepers located on the exterior meeting rail.

Impact Resistance: These window assemblies do not satisfy the Texas Department of Insurance's
criteria for protection from windborne debris. These window assemblies will not need to be protected with
an impact protective system.
Acceptance of Smaller Assemblies: Windows assemblies with dimensions equal to or smaller than
those specified above are acceptable within the limitations specified in this report.
INSTALLATION INSTRUCTIONS
General: The window assembly shall be prepared and installed in accordance with the manufacturers
recommended installation instructions. Detailed installation instructions and drawings are available from
the manufacturer.
Installation:
System 1: The wall framing shall be minimum Spruce -Pine -Fir dimension lumber. The window is
secured to the wall framing members using the window frame head and side jambs with minimum No.
8 x 2" screws. The fasteners are placed at the following locations: one at the top and bottom of each
jamb, two at the approximate mid -span of each jamb, and one approximately 6 inches from each end
of the head. All fasteners shall be long enough to penetrate a minimum of 1 y inches into the wall
framing.

Applicant shall leave window data stickers in place until a city inspection can
confirm proper window type and installation. Applicant/owner shall be
responsible for requesting the required inspection.

RESIDENTIAL ENERGY EFFICIENCY
SECTION 401
GENERAL
401.1 Scope. This chapter applies to residential buildings
401.2 Compliance. Projects shall comply with Sections 401,
402.4, 402.5, and 403.1, 403.2.2, 403.2.3, and 403.3 through
403.9 (referred to as the mandatory provisions) and either:
1. Sections 402.1 through 402.3, 403.2.1 and 404.1 (pre-
scriptive); or
2. Section 405 (performance).
401.3 Certificate. A permanent certificate shall be posted on or
in the electrical distribution panel. The certificate shall not cover
or obstructthe visibility of the circuit directory label, service dis-
connect label or other required labels. The certificate shall be
completed by the builder or registered design professional. The
certificate shall list the predominant R-values of insulation
installed in or on ceiling/roof, walls, foundation (slab, basement
wall, crawlspace wall and/or floor) and ducts outside condi-
tioned spaces; U-factors for fenestration and the solar heat gain
coefficient (SHGC) of fenestration. Where there is more than one
value for each component, the certificate shall list the value cover-
ing the largest area The certificate shall list the types and efficien-
cies of heating, cooling and service water heating equipment
Where a gas -fired unvented room heater, electric furnace, or base-
board electric heater is installed in the residence, the certificate
shall list "gas -fired unvented room heater," "electric furnace" or
"baseboard electric heater," as appropriate. An efficiency shall not
be listed for gas -fired umentedroom heaters, electric furnaces or
electric baseboard heaters.

BUILDING THERMAL ENVELOPE
402.1 General (Prescriptive).
401.1.1 Insulation and fenestration criteria. The building
thermal envelope shall meet the requirements of Table
402.1.1 based on the climate zone specified in Chapter 3.
402.1.2 R-value computation. Insulation material used in
layers, such as framing cavity insulation and insulating
sheathing, shall be summed to compute the component
R-value. The manufacturer's settled R-value shall be used
for blown insulation. Computed R-values shall not include
an R-value for other building materials or air films.

A limited asbestos survey was conducted of the King's Bay Inn motel at 1601 S Broadway in Laporte,
TX. The survey was conducted on July 1, 2010 by Robert L. Voorhies, a Texas Dept. of State Health
Services (DSHS) licensed asbestos consultant (DSHS License #10-5524). A total of 8 asbestos samples
were collected and analyzed. Mr. Voorhies is employed by ASTE, LLC, a DSHS licensed Asbestos
Consulting Agency (DSHS license # 10-0019).
All asbestos samples were analyzed in the Texas Dept. of State Health Services (DSHS) licensed (#30-
0080) and EPA/NVLAP accredited laboratory (# 101793-0) of A & B Environmental Services, Inc. at
their facility at 10100 East Freeway Suite 100, Houston, Texas. The EPA Method 600/R-93/116, July,
1993, was used for asbestos analysis. Components of non -homogeneous samples were analyzed and
reported separately. Percentages of asbestos reported are based on calibrated microscopic visual
inspection.
FINDINGS
No asbestos was found in the samples of sheetrock joint compound/ wall texture or ceiling texture in the
areas tested. No other untested suspect materials were seen inside the areas inspected. Based on these
findings the planned renovations of the guest rooms may be done without additional asbestos related
requirements. All materials in the areas tested may be treated as ordinary non -asbestos building
materials and may be removed and disposed of as unregulated construction debris.
A complete list of all sample locations, types and analytical results is on the following "Summary of
Analytical Results". The laboratory analytical reports and chains of cuslpdgo included at the end

The samples were collected in a manner that reduced potential for fiber release and exposure by using
wet sampling methods and personal protective equipment, as necessary. Sampling equipment was wet
wiped and decontaminated after each use to prevent cross contamination of the samples. All samples
were deposited into secure containers for transport to the laboratory of A & B Environmental Services,
Inc. in Houston, TX.
The project consisted of inspection and sampling of suspect asbestos containing materials in the areas
specified by the Client to allow for required renovations. The intent of the limited inspection was to
locate and identify significant quantities of asbestos containing materials. The inspection was conducted
in conformance with current State DSHS regulations, which require a minimum of 3 samples collected
of each different suspect asbestos containing material. For larger areas additional samples may be
collected to follow recommended Federal AHERA guidelines.
The motel is a relatively small 2 story building with all rooms accessible from the front of the building.
The owners reported it was built in about 1980. They are unaware of any significant remodeling. The
planned remodeling is primarily painting and texturing of existing sheetrock and installing new ceramic
floor tiles and carpet. Some disturbance of sheetrock walls and ceilings may be required for plumbing
upgrades.
All of the rooms inspected had similar wall and ceiling materials (texture on sheetrock). In many of the
rooms, which were not currently rentable, the floors were bare concrete. In the 2 rentable rooms
inspected the floors were carpet in the sleeping area and ceramic tile in the bathroom area. In the non -
rentable rooms the ceiling texcture was trhe same as on the walls. In the 2 rentable rooms inspected,20
and 23, the ceiling texture was painted "popcom" type texture. The Owners indicated there was no
vinyl floor tile or linoleum in any of the rooms. The exterior walls are wood with no suspect materials
visible.
Photos of the building and various areas and suspect materials are included in this report.
A total of 8 samples, 5 of the joint compound and 3 of ceiling texture, were collected, analyzed and
reported. Samples were collected in 5 different rooms. A minimum of 3 samples were collected of each
of the suspect materials to comply with State regulations and AHERA guidelines.
All samples collected were analyzed and reported. A complete list of all sample locations, types and
analytical results is on the following "Summary of Analytical Results". The laboratory analytical
reports and chains of custody are also included.

RECOMMENDATIONS FOR FURTHER ACTION
No asbestos was found in the samples of sheetrock joint compound/ wall texture or ceiling texture in the

POLARIZED LIGHT MICROSCOPIC ANALYSIS
The asbestos samples were analyzed in the laboratory of A & B Environmental Services, Inc. using a
Nikon Labophot POL or Olympus 131-1-2 microscope. Standard oil immersion and optical staining
techniques were used. The EPA Method for the Determination of Asbestos in Bulk Building Materials
(EPA/600/R-93/116, July, 1993) was followed in the analyses. A & B Environmental Services, Inc. is
licensed by the Texas Dept -of -State Health Services (License # 30-0080) and is an EPA/NVLAP
accredited laboratory (# 17'3) forithe analy/of bulk samples for asbestos by Polarized Light

SURVEY LIMITATIONS
The survey performed on the subject property was a limited scope survey designed to identify the major
occurrences of ACM inside the area specified. This limited survey made no attempt to locate or identify
all occurrences of ACM inside or on the exterior of the buildings, nor does this report guarantee that
additional ACM is not present. This survey was limited to accessible areas in the areas specified. Areas
such as, but not limited to, inaccessible rooms, inside the walls and beneath the existing flooring,
including carpeting and multi -layered tile and/or linoleum, are specifically excluded from the scope of
this survey. At the Client's instructions the restrooms were not sampled. The materials were sampled in
accordance with State requirements for inspection of interior areas. If, during renovation or demolition
activities previously undiscovered suspect materials are found, work in that area must immediately cease
until the material can be checked by a DSHS licensed inspector.

For good consideration I the under sigh jointly and severally hereby for
ever release , discharge, acquit and for give Guadalupe Lopez plumbing co.
and Guadalupe Lopez JR from any and all claims, action, suits, demands,
agreements, and each of them, if more than one labiality , judgments, and
proceedings both at law and equity arising from the beginnings of time to
the date of these presents and as more particularly related to or arising from
Guadalupe Lopez Plumbing co. and Guadalupe Lopez JR being terminated
for taking to much time to install under ground Plumbing due to the fact that
old existing Plumbing where new connections have to be made. Are at
another location out side building.

outside building. To locate old existing building sewer line to hook up new
plumbing. Unfortunately this took more away from the original work of the
regular contract work time. So MR GUADALUPE LOPEZ JR gave MR
ISHVAR BHAKTA back $ 400,00 dollars back at the request of MR
ISHVAR BHAKTA SO HE COULD HIRE A NEW LICENED PLUMBING TO
COMPLETE WHAT IS LEFT OF THE WORK NEED TO FINISH THIS JOB. MR
GUADALUPE LOPEZ JR AND GUADALUPE LOPEZ PLUMBING CO.
HAVE COMPLETED 95 % OF THE GROUND WORK AND TOP OUT
ON THIS TWO UNITS WITH FIRST DRAW OF THE ORIGINA
CONTRACT. At time of signing this release for MR GUADALUPE
LOPEZ PLUMBING CO and GUADALUPE LOPEZ JR will remove this
Plumbing Permit for this job and for all work and responsibility at this
