All work was performed in accordance with the scope of environmental consulting services outlined in our proposal. Our services included the investigation for possible suspect asbestos -containing materials (ACM) and, if applicable, subsequent collection of bulk material samples and the corresponding evaluation of the material(s). Suspect ACM were discovered and samples were collected to establish if asbestos is present in the materials. If the laboratory analysis indicated asbestos amounts greater than 1 %, a physical assessment and response action are listed. FIELD FINDINGS Ms. Christy Smith conducted the limited asbestos survey on January 27, 2014. Ms. Smith is licensed as an Asbestos Inspector (license no. 60-2202) and her license is included in Appendix C. A total of twenty-three bulk asbestos samples (43 layers) were collected from seven homogeneous materials. None of the seven materials sampled contained greater than 11/c asbestos. In accordance with the Environmental Protection Agency, maer1IscWitaining greater than 1% asbestos are considered asbestos -containing materials. r-C ) .J The following suspect asbestos -containing building materials SWreesafnpled during the survey: ➢ 2'x4' White Ceiling Tile (asbestos not detected in this material) ➢ Blue Baseboard/Mastic (asbestos not detected in this material) ➢ White Baseboard/Mastic (asbestos not detected in this material) ➢ 12"xl2" Beige Floor Tile with Speck Patt It FoRWRO/'i@(@lldlfl&A"detected in this material) ODIPIED, A NEW SUBMITTAL T ECI' ,c ➢ Floor Tile with Yellow Glue beneath 12" e,rI r49 3tgg"detected in this material) P (AAGE t CCURRING. ➢ Sheetrock/Joint CompoundlTexture (asbestos not detected in this material) Total Safety, U.S., Inc., d/b/a EHS Services 4100 North Sam Houston Parkway West Suite 290, Houston, TX 77086-1467 ' 2300 Highway 365, Suite 370, Nederland, TX 77627 Office Ouilding -'Suite 100 1200 Highway 146, LaPorte, TX Limited Asbestos Survey Report January 2014 Yellow Carpet Glue (asbestos not detected in this material) LABORATORY ANALYSIS All bulk samples collected were submitted to EMSL Analytical, Inc. (EMSL) for analysis. The samples were analyzed by polarized light microscopy (PLM) in compliance with guidelines established by the USEPA (50 CFR Part 763.115). EMSL is a successful participant in the National Institute of Standards and Technology's National Voluntary Laboratory Accreditation Program (NVLAP). EMSL's NVLAP license number is 102106. EMSL's Department of State Health Services license number is 30-0159. SAMPLE RESULTS Asbestos was not detected in amounts greater than the regulatory standard in the materials sampled during this survey. The Bulk Asbestos Sample Table is located in Appendix A. The laboratory results are included as Appendix B. CLOSING The asbestos survey indicated that no asbestos above the regulatory standard is present in the materials sampled at Suite 100 at the office building located at 1200 Highway 146 in La Porte, Texas. We appreciate the opportunity to provide these services and look forward to working with you in the future. Should you have any questions, feel free to contact me at (409) 727-8227.
