(1.) No direct connection between the public drinking water supply and a potential source of Compliance Non -Compliance
contamination exists. Potential sources of contamination are isolated from the public water tl/

Occupancy issued, We further understand that the temporary utilities may be disconnected for any violation
of the Code. The General Contractor and owner assume the responsibility for any violation of any city
ordinance by the subcontractor.
Temporary electrical permits are in force for 3 months (90 days). If more time is required to complete the
building, a new application shall be submitted by a licensed electrical contractor and be accompanied by the
temporary electrical permit fee.
The undersigned hereby agrees to Indemnify and hold forever harmless the City of La Porte, its officers, and

3) The Fire Marshal's Office must witness a function test of the Fire Alarm system. Contact
the Fire Marshal's Office 24hrs in advance to schedule a Fire Alarm system test.
4) Approved (Stamped) copy of the Fire Alarm plans shall be on the job site and available to
the Fire Marshal Inspector at any time during installation.

20. Contractor/owner shall be responsible for ensuring all construction materials/debris

21. Owner/developer of commercial driveways in concrete shall be constructed 12" from the
adjacent asphalt street. The contractor shall clean the area between the driveway and
the street to a depth of at least 6". It is the responsibility of the

To whom it may concern:
Maverick Testing Laboratories will be qualifying welds to a variety of different
codes such as API, ASME and AWS. It will consist of a welder coming to our shop
where we will have test booths set up and have the test coupons machine
beveled, which we will also be doing there, and once he has completed his
welding test, approximately 2 hours, the test coupons will be destructively tested.
We will saw the coupons into sections and mill them down according to the code
and then they will go to our lab where they will be tested.

1.0 INTRODUCTION
QC Laboratories, Inc. (QC) conducted a limited asbestos survey at the Underwood Business Park
located in La Pone, Texas. Bulls samples of suspect Asbestos -Containing Building Materials
(ACBM) were collected from the interior of the SPEC 14 ® 10001 Porter Road space only. The
scope of work included: Review of previous documentation (if provided), inspecting and sampling
for suspect ACBM's.
No destructive sampling was conducted during this survey. The exterior, including the roof, of the
building was excluded.
2.0 SCOPE OF SERVICES
Samples were collected and analytical tests were performed for the purpose of determining whether
ACBM were present in the facility. The survey follows applicable state and federal regulations
governing asbestos work and can be relied on for renovation activities.
QC conducted a visual inspection of the building materials. During the walk-through, suspect
ACBM were noted. Their condition and friability were also noted.
With regard to asbestos, a homogeneous area (an area of material that is uniform in color and texture)
shall be detemuned to contain asbestos if the results of at least one sample collected from it indicate
that asbestos is present in an amount greater than one percent (> 1%). Once a material has been
found to contain asbestos, all such materials in the defined homogenous area are considered to contain
asbestos.
3.0 SAMPLING
Mr. Mustafa Birader, a TDSHS asbestos inspector (license #60-2369), conducted the site sampling
survey on October 8, 2010 in accordance with Texas Asbestos Health Protection Rules, §295. A
total of twenty-one (21) bulk samples of suspect ACBM's were collected. The materials sampled,
sample location, and analytical results are included in Table 1.
Suspect ACBM, which are alike in appearance and application, were sampled as a homogeneous
area. Suspect homogeneous areas are divided into three classifications:
1. Surfacing materials: Spray -applied or troweled on material.
2. Thermal Systems Insulation: Pipe, boiler, tank, or flue insulation.
3. Miscellaneous: Other suspect material, including floor tile, floor the mastic, sheet vinyl
flooring, ceiling tile and panel, insulation mastics, cove base mastic, window
caulking/glazing, and exterior siding (transite).
Suspect materials sampled and analyzed should be considered homogeneous area iE

Bulk samples were analyzed under polarized light microscopy (PLM) by J3 Resources, Inc.,
Houston, Texas, utilizing the Environmental Protection Agency's Method for the Detection of
Asbestos in Bulk Insulation Samples, (EPA 600/R-93 July 1993). J3 resources, Inc. is accredited by
the National Voluntary Laboratory Accreditation Program (NVLAP), participates in the NVLAP
and AIHA Bulk Asbestos Sample Quality Assurance Programs and is licensed to analyze bulk
asbestos samples collected in the State of Texas.
5.0 ANALYTICAL RESULTS
No asbestos was detected in any of the samples collected as part of this inspection. Table I,
included with this report, contains a detailed list of suspect asbestos materials sampled as part of this
project.
6.0 CONCLUSIONS AND RECOMMENDATIONS
If renovation activities discover suspect asbestos materials that were not included in the scope of
this project, they should be sampled and analyzed for asbestos content. If the exterior of the
building, including the roof, is to be renovated, then the building exterior should be inspected for
asbestos.

7.0 LIMITATIONS
The conclusions presented in this report are professional opinions based solely upon visual
observations of the site, at the time of our investigation, and the results of laboratory analysis. The
opinions presented herein apply to site conditions existing at the time of our investigation and those
reasonably foreseeable. Quantity estimates of confirmed ACBMs are preliminary, based on
observations made during our survey and should not be used to prepare a removal cost estimate.
No destructive sampling was conducted during this survey. Un-sampled asbestos -containing
construction materials may be located within walls, ceiling cavities, below flooring or grade, and
other non -accessible areas. Precaution should be used in relation to these un-sampled materials until
their asbestos content has been detemilned by proper sampling and analysis. The condition of the
ACBMs may change gradually or suddenly, depending upon use, maintenance or accident.
