On behalf of the Quala Services, LLC (Quala), The WCM Group, Inc. (WCM) is submitting the
above referenced documentation and Wastewater Treatment System (WWTS) drawings in
support of the proposed "like and kind" replacement of two (2) existing 60,000-gallon aboveground
storage tanks associated with the above referenced facility (See Figure 1 — Plot Plan). The
WWTS is depicted in Figure 2 (See Figure 2 — WWTS Details). Quala is proposing to replace the
two (2) existing tanks with two (2) tanks of identical material composition and capacity and will
not alter or modify the current wastewater treatment operations per the attached design
information (See Attachment A). Per your request, a Draft submittal of the Permits -By -Rule (PBR)
documentation (to be submitted to the TCEQ) is included (See Attachment B). The proposed
replacement will not require changes to existing electrical or piping at the facility.
For the replacement, the existing tanks will be taken out of service, de -inventoried, dismantled
and removed from the site. The replacement tanks will be installed in the same location as the
existing tanks. During this replacement period, frac tanks will be mobilized on -site and used as
temporary wastewater storage vessels until the replacement tanks can be placed into service
(See Figures 3 and 3A — Wastewater Treatment System Flow Diagram). The frac tanks are
scheduled to arrive on -site the week of August 2911 such that the temporary piping can be
installed. The decommissioning of the existing wastewater tanks to be replaced is anticipated to
begin the week of December 51h. The replacement tanks are scheduled to begin commissioning

1. These calculations provide for the structural design of all major tank components. Please
contact Chubb Engineering, LLC for design of any structural component not specifically included
within these calculations.
2. These calculations do not extend to any process design, including but not limited to the
chemical or electro mechanical corrosion potential, coating, or hardware compatibility with the
stored product or the environment.
3. Chubb Engineering, LLC and the design engineer are not responsible for the design of any
portion or component of the tank that is not specifically included in these calculations.
4. All attachments and clips are to be bolted, welded, or otherwise properly connected to
develop the full strength of such attachments and clips.
S. Chubb Engineering, LLC shall be provided written notice and, if appropriate, Chubb
Engineering, LLC shall approve any modifications to the original tank including but not limited to
(a) installation of additional openings; (b) modifications to openings; (c) attachments of
equipment; (d) application of external or internal forces not included in the original calculations;
(e) changes in service life; or (f) any other deviations from the original design intent.
6. Tank foundations shall be level, within 1/8" in any 30' circumference under the shell.
Levelness on the circumference shall not vary more than 1/4" from the established plane.
7. It is the customer's responsibility to ensure that proper tank ventilation is provided and
maintained at all times.
8. As defined herein, freeboard is the distance from MOL to the tank eave. Actual freeboard as
defined by AWWA is the distance from MOIL to the lowest level of roof framing.
9. Tank is to be supported on one of the Foundation Types described in AWWA D103.
10. All structural welding is to be performed by a certified welder using qualified procedures.

On behalf of the Quala Services, LLC (Quala), The WCM Group, Inc. (WCM) is submitting the
above referenced documentation and Wastewater Treatment System (WWTS) drawings in
support of the proposed `like and kind" replacement of two (2) existing 60,000-gallon aboveground
storage tanks associated with the above referenced facility (See Figure 1 — Plot Plan). The
WWTS is depicted in Figure 2 (See Figure 2 — WWTS Details). Quala is proposing to replace the
two (2) existing tanks with two (2) tanks of identical material composition and capacity and will
not alter or modify the current wastewater treatment operations per the attached design
information (See Attachment A). Per your request, a Draft submittal of the Permits -By -Rule (PER)
documentation (to be submitted to the TCEQ) is included (See Attachment B). The proposed
replacement will not require changes to existing electrical or piping at the facility.
For the replacement, the existing tanks will be taken out of service, de -inventoried, dismantled
and removed from the site. The replacement tanks will be installed in the same location as the
existing tanks. During this replacement period, frac tanks will be mobilized on -site and used as
temporary wastewater storage vessels until the replacement tanks can be placed into service
(See Figures 3 and 3A — Wastewater Treatment System Flow Diagram). The frac tanks are
scheduled to arrive on -site the week of August 29" such that the temporary piping can be
installed. The decommissioning of the existing wastewater tanks to be replaced is anticipated to
begin the week of December 51". The replacement tanks are scheduled to begin commissioning V \1

1. These calculations provide for the structural design of all major tank components. Please
contact Chubb Engineering, LLC for design of any structural component not specifically included
within these calculations.
2. These calculations do not extend to any process design, including but not limited to the
chemical or electro mechanical corrosion potential, coating, or hardware compatibility with the
stored product or the environment.
3. Chubb Engineering, LLC and the design engineer are not responsible for the design of any
portion or component of the tank that is not specifically included in these calculations.
4. All attachments and clips are to be bolted, welded, or otherwise properly connected to
develop the full strength of such attachments and clips.
S. Chubb Engineering, LLC shall be provided written notice and, if appropriate, Chubb
Engineering, LLC shall approve any modifications to the original tank including but not limited to
(a) installation of additional openings; (b) modifications to openings; (c) attachments of
equipment; (d) application of external or internal forces not included in the original calculations;
(e) changes in service life; or (f) any other deviations from the original design intent.
6. Tank foundations shall be level, within 1/8" in any 30' circumference under the shell.
Levelness on the circumference shall not vary more than 1/4" from the established plane.
7. It is the customer's responsibility to ensure that proper tank ventilation is provided and
maintained at all times.

Qualawash Holdings, LLC (QHL) operates an existing tank container cleaning facility located at
11110 Highway 225, La Porte, Texas (RN102993078) under TCEQ Air Permit No. 9723.
Wastewater is generated from the internal and external cleaning of empty tank containers that
may have transported any of a variety of materials. It has been identified that the wastewater
contains small quantities of organic compound residues removed from container surfaces.
This document demonstrates authorization for modification of the wastewater handling system.
Specifically, the wastewater effluent and influent storage tanks of the permitted wastewater
treatment system are proposed for replacement with two new "like and kind" tanks constructed of
the same material and with same dimensional capacities. The anticipated date for proposed tank
replacement is the week of December 05, 2016. A separate correspondence will be submitted
as required, no later than ten days after completion of the proposed installation Represented in
the tables below are the units that qualify for authorization under TCEQ Permit -By -Rule 30 TAC
106.264 (Replacement of Facilities).

2.0 PBR COMPLIANCE
The existing wastewater treatment vessels function as wastewater storage units and are
authorized by Air Quality Permit No.9723. The replacement of the wastewater treatment vessels
with like and kind equipment is being authorized by PBR 106.264. A checklist demonstrating
compliance with the requirements of 106.4 - Requirements for Permitting -By -Rule and 106.264 —
Replacements of Facility is provided in the Attachment B of this document.
This registration addresses the replacement wastewater treatment vessels currently authorized
use as of stationary aboveground tanks for storage of wastewater generated from the container
cleaning operation. Associated with wastewater storage is the periodic transfer of the wastewater
to transport vessels for off -site shipment. These emission sources are authorized under TCEQ

2.1 PERMIT BY RULE 4106.264
A facility which replaces an existing facility is permitted by rule provided that the following
conditions of this section are satisfied:
(1) the replacement facility functions in the same or similar manner as the facility to be
replaced;
(2) the emissions from the replacement facility are not more than nor have different
characteristics than those from the facility to be replaced;
(3) the emissions from the replacement facility will not exceed 25 tons per year of any air
contaminant;
(4) the physical location of the replacement facility is the same or immediately adjacent to the
facility being replaced;
(5) there will be no increase in capacity, production rate, or throughput as a result of the
replacement;
(6) notwithstanding the provisions of paragraph (3) of this section, the emissions from the
replacement facility will not contain any compounds (other than carbon monoxide, nitrogen
oxide, or sulfur dioxide) listed or proposed to be listed as hazardous constituents in 40
Code of Federal Regulations 261, Appendix VIII;
(7) notification of the replacement is provided to the executive director within ten days
following installation of the replacement facility.

Title 30 Texas Administrative Code § 106.4
The following checklist was developed by the Texas Commission on Environmental Quality (TCEQ), Air Permits
Division, to assist applicants in determining whether or not a facility meets all of the applicable requirements. Before
claiming a specific Permit by Rule (PBR), a facility must first meet all of the requirements of Title 30 Texas
Administrative Code S 106.4 (30 TAC § 106.4), "Requirements for Permitting by Rule." Only then can the applicant
proceed with addressing requirements of the specific Permit by Rule being claimed.
The use of this checklist is not mandatory; however, it is the responsibility of each applicant to show how a facility being
claimed under a PBR meets the general requirements of 30 TAC § 106.4 and also the specific requirements of the PBR
being claimed. If all PBR requirements cannot be met, a facility will not be allowed to operate under the PBR and an
application for a construction permit may be required under 30 TAC § 1 16.110(a).
Registration of a facility under a PBR can be performed by completing Form PI4 (Registration for Permits by Rule) or
Form PI-7-CERT (Certification and Registration for Permits by Rule). The appropriate checklist should accompany the
registration form. Check the most appropriate answer and include any additional information in the spaces provided. if
additional space is needed, please include an extra page and reference the question number. The PBR forms, tables,
checklists, and guidance documents are available from the TCEQ, Air Permits Division Web site at:

1. These calculations provide for the structural design of all major tank components. Please
contact Chubb Engineering, LLC for design of any structural component not specifically included
within these calculations.
2. These calculations do not extend to any process design, including but not limited to the
chemical or electro mechanical corrosion potential, coating, or hardware compatibility with the
stored product or the environment.
3. Chubb Engineering, LLC and the design engineer are not responsible for the design of any
portion or component of the tank that is not specifically included in these calculations.
4. All attachments and clips are to be bolted, welded, or otherwise properly connected to
develop the full strength of such attachments and clips.
5. Chubb Engineering, LLC shall be provided written notice and, if appropriate, Chubb
Engineering, LLC shall approve any modifications to the original tank including but not limited to
(a) installation of additional openings; (b) modifications to openings; (c) attachments of
equipment; (d) application of external or internal forces not included in the original calculations;
(e) changes in service life; or (f) any other deviations from the original design intent.
6. Tank foundations shall be level, within 1/8" in any 30' circumference under the shell.
Levelness on the circumference shall not vary more than 1/4" from the established plane.
7. It is the customer's responsibility to ensure that proper tank ventilation is provided and
maintained at all times.
8. As defined herein, freeboard is the distance from MOL to the tank eave. Actual freeboard as
defined by AW WA is the distance from MOL to the lowest level of roof framing.
9. Tank is to be supported on one of the Foundation Types described in AW WA D103.
10. All structural welding is to be performed by a certified welder using qualified procedures.
