I am not aware of any life safety issues. Therefore I see no reason to not issue a CO. The A/C as it currently stands is a matter outside of the contractor's control. The contractor has not submitted the closeout documentation and until this happens the City will not release retainage. The survey. was performed in accordance with Senate Bill 509 and to comply with the U.S. EPA's National Emissions Standard for Hazardous Air Pollutants regulations (known as NESHAP, 40 CFR 61, Subpart M) and the Texas Asbestos Health Protection Rules (known as TAHPR, 25 TAC 295.31 - .71). Mr. Ronald Fife of CAM Environmental Services (CAM) performed the limited asbestos survey on July 13, 2006. The following is the report of findings from the sampling conducted. Included with this report are (1) Polarized Light Microscopy (PLM) Laboratory Results; (2) Chain of Custody Forms; and, (3) Certificates and Licenses. SCOPE OF WORK i The survey included identification and bulk sampling of the homogenous areas suspected to contain asbestos. A homogenous area (HA) is an area of surfacing material, thermal system insulation, or miscellaneous material that is uniform in color and texture, apparently applied at similar times, and appears identical in every other respect:' I SUSPECT MATERIALS SUMMARY ,During the asbestos sampling, eighteen (18) samples were collected from six (6) homogenous areas within the building. Refer to the attached Asbestos Bulk Sample Analysis for sample descriptions and locations. It is possible that additional suspect materials may be found in the building area during renovation/demolition that were not accessible during the time CAM collected the asbestos bulk samples. RECOMMENDATIONS Current regulations specify that a homogeneous sampling area (HSA) be considered.as an ACM if any one (1) of the samples obtained from that area is found to contain asbestos. In this regard, once one (1) sample is found to contain more than one percent (1%) asbestos, the remaining samples obtained from that particular homogeneous area must also be considered ACM. All of the samples obtained from a homogeneous area must be 'analyzed and found to contain equal to or less than one percent (1%) asbestos before that area can be considered non -ACM. It is CAM's understanding that there will be renovation activities performed within the subject area. Prior to these activities, state and federal laws require the removal of all friable and non -friable ACM that may be disturbed during the renovation activities by a licensed asbestos abatement contractor with third party independent consulting and air monitoring. If any additional suspect materials other than those contained in this report are found during the proposed renovations, these materials should be tested immediately. LABORATORY ANALYSIS: QA/QC Asbestos bulk analysis was conducted by CAM's laboratory in Pasadena, Texas. Samples were analyzed by Polarized Light Microscopy with Dispersion Staining, EPA Method 600/-80-020. CAM's laboratory is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP # 200240-0) for asbestos bulk and fiber analysis. The analysis for asbestos fibers consisted of the determination of the optical properties such as morphology,•color, refractive indices and other properties as outlined in Appendix A, Subpart F, 40 CFR part 763, Section 1, Polarized Light Microscopy. An asbestos containing material is defined to be one that contains greater than one percent (1%) asbestos. Portions of each bulk material were immersed in known refractive index oil and compared to known asbestos types, for identification and characterization. Additionally, the EPA indicates that further analysis by point counting be performed to confirm asbestos content for friable materials found to contain less than ten percent (10%) asbestos by polarized light microscopy. Point counting analysis maybe omitted if the material is assumed to contain more than one percent (1%) asbestos and is then handled accordingly. In general, because of the limitations of the analytical methodology, and our experience with historical data from local laboratories, any material found to contain more than approximately four percent (4%) asbestos is not generally recommended for point counting. Optical estimates were made by comparing the quantity of non -asbestos material to asbestos fibers. Quality assurance and quality control procedures were implemented at the completion of the inspection to ensure that reliable sampling, analysis, and documentation occurred. CAM's Laboratory Director reviewed all analytical procedures. CAM's laboratory performs QA/QC on asbestos bulk samples by duplicating the analysis on ten (10%) of daily samples for accuracy.
