This report presents the results of the limited asbestos sampling in the Former Kroger Grocery Store
located,at 201 West Fairmont Parkway in La Porte, ,Texas..The inspection was, performed in,accordance
with. 1 n'afe Bill 509 and.'to.comply with the U,$. EPA's,Nabonal,Emissions Standard for Hazardous Air
Pollutants regulations,(known as NESHAP, 40 CFR 61„Subpart M), and,the .Texas Asbestos, Health
Protection Rules (known as TAHPR, 25 TAC 295.31 - .71). Mr. Jimmy Farrier of CAM Envi�onmenta(
Services Pasadena office perforrned.the.asbestos inspection on September,29; 2009. All sampling was
conducted utilizirig ,the accepted Asbestos Hazard.Emergency Response Act (AHERA) protocol specified
in 40 CFR 763.85-763.88.
The-followind' is the report of 'findings from the sampling conducted. Included with this report are (1)
Polarized Light Microscopy (PLM) Laboratory Results; (2) Chain of Custody Forms; (3) DSHS Certificates
and"Licenses:
SCOPE OF WORK
At the`dir6d'on 'of the'Malachite`Group, the areas inspected'as partof this'asbestos-inspection were
limited`to the"Fom er'Kroger Grocery Store's'interior'sheetrock components and acoustical ceiling tiles
prior to planned demolition activities. Interior flooring materials and exterior component were not included
withinCAM's scope 'of work."
The" inspection'included identification and'bulk sampling- "of the honiogenous'areas suspected to contain
asbestos A `homogenous 'area (HA) is an area of 'srirfacing- material, thermal system' insulation, or
miscellaneous` material'that'-is.uniform in color and texture; 'apparently applied at'similar times, and
appearsidentical in everyotFier respect.'
SUSPECT MATERIALS SUMMARY
During the asbestos sampling,six,(6) material. samplesvere collected from two (2) homogenous areas in
the Former Kroger Grocery Store located ats201 West -Fairmont Parkway in La Porte,, -Texas. The

Out of the six (6) material samples collected and analyzed for asbestos content, none of the samples
were found to contain asbestos greater than 1.0%. It is possible that additional suspect materials may
befioundin' the building area during demolition that was not accessible during the time CAM collected
samples.

Current regulations specify that a homogeneous sampling area (HSA) be considered as an ACM if any one
of thessamples obtainedfrom that:area•is foundto contain asbestos: In this_regaro once; one (1).sample is
found,.torcontain-more than one percent (1"/0) asbestos, -the remaining -,samples obtained from • that:particular
homogeneous area must also be considered ACM. -All of:the .samples obtained from.a homogeneous area
mustbe analyzed and-found,tacontaimequal to or less than one'percent (1%) asbestos before that -area can
be considered non -ACM.

demolition process) by a TDSHS licensed abatement contractor with third party independent consulting
and,air.monitoring .bya:TDSHS licensed asbestos,.consultant.: if any, additional suspect materials other
thanAhose contained.in this report: are found -during the demolition activities, these materials should be
tested immediatelyby:a,TDSHS licensedasbestos:inspector.
LABORATORY ANALYSIS: QA/QC
Asbestos bulk analysis was conducted by CAM's laboratory in Pasadena, Texas. Samples were
analyzed by Polarized Light Microscopy with Dispersion Staining, EPA Method 600/-80-020. CAM's
laboratory is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP # 200240-0)
for asbestos bulk and fiber analysis. The analysis for asbestos fibers consisted of the determination of
the optical properties such as morphology, color, refractive indices and other properties as outlined in
Appendix A, Subpart F, 40 CFR Part 763, Section 1, Polarized Light Microscopy.
An asbestos containing material is defined to be one that contains greater than one percent (1%)
asbestos. Portions of each bulk material were immersed in known refractive index oil and compared to
known asbestos types, for identification and characterization. Additionally, the EPA indicates that further
analysis by point counting be performed to confirm asbestos content for friable materials found to contain
less than ten percent (10%) asbestos by polarized light microscopy. In general, because of the limitations of
the analytical methodology, and our experience with historical data from local laboratories, any material
found to contain more than approximately four percent (4%) asbestos is not generally recommended for �.
point counting.
Optical, estimates were made by comparing the quantity of non -asbestos material to asbestos fibers.
Quality assurance and quality control procedures were implemented at the completion of the inspection to
ensure, that reliable sampling, analysis, and documentation occurred. CAM's Laboratory Director
reviewed all analytical procedures. CAM's laboratory performs QA/QC on asbestos bulk samples by
duplicating the analysis on ten percent (10%) of daily samples for accuracy.
METHODOLOGY AND.SAMPLING
The asbestos inspection was conducted utilizing acceptable protocols for Public Buildings as set forth in
the current Texas Asbestos Health Protection Rules (TAHPR) Section 295.58, paragraph (h). All suspect
asbestos materials to be disturbed for sampling purposes were fully wetted with amended water prior to
sampling. Destructive sampling was not performed. Asbestos containing materials are sometimes found
in areas that are inaccessible at the time the sampling is performed. Suspect materials were sent for
PLM analysis. Only those persons necessary for sampling were allowed in the a a� Sin I were
collected to obtain III layers of the suspect materials to the substrate.

LIMITATIONS
CAM Environmental Services (CAM) warrants that services provided shall be performed in accordance
with established and recognized general industry standards and analytical testing procedures. No other
warranties, expressed or implied, are made by CAM.
The scope of this report is limited to matters expressly covered. All recommendations, findings and
conclusions stated in this report are based upon the facts and circumstances, as they existed at the time
that this report was prepared,(e.g;wFederal; State, arid, Local laws;;rules, regulations and general industry
standards). A change in any factor circumstances upon which this report is based may adversely affect
the recommendations, findings, and conclusions expressed in this report.
The conclusions and recommendations describe only the conditions present at the time of our survey that
could be observed in accessible areas. Other similar conditions may exist in inaccessible areas, such as
behind walls, above permanent ceilings or beneath flooring.
CAM does not warrant or guarantee any quantity estimations or cost estimations presented in this report.
Although CAM believes estimates presented in this report are reasonable, the use of such estimates shall
be at the user's own risk and shall constitute a release and agreement to defend and indemnify CAM from
and against any liability in connection with cost and/or quantity estimations.
