5. ALL WORK SHALL BE CONDUCTED WITHIN THE RIGHT -OF WAY AND/OR EASEMENTS SHOWN UNLESS OTHERWISE
APPROVED BY THE OWNER AND/OR ENGINEER.
6. CONTRACTOR IS RESPONSIBLE FOR STAKING AND LAYOUT OF THE WORK IN ACCORDANCE WITH THE PLANS AND
FURNISHED CONTROL POINTS.
7. EXISTING PROPERTY CORNERS ARE NOT TO BE DISTURBED. THOSE REMOVED DURING CONSTRUCTION SHALL BE
RESET UNDER THE SUPERVISION OF A REGISTERED SURVEYOR AT NO COST TO OWNER.
8. EXCESS EXCAVATED MATERIAL SHALL BE DISPOSED OF PROMPTLY. CITY HAS THE OPTION OF TAKING ANY SALVAGED
MATERIAL AS DEEMED NECESSARY.
9. CONTRACTOR SHALL ASSIST OWNER IN NOTIFYING BUSINESS ESTABLISHMENTS OF ANY SERVICE INTERRUPTIONS.
10. CONTRACTOR SHALL TAKE CARE WHEN WORKING WITHIN PERMANENT AND WORKING EASEMENTS. ALL DAMAGED
PLANTS, TREES, SHRUBBERY, FENCE AND STRUCTURES WHICH ARE TO REMAIN SHALL BE REPAIRED OR REPLACED
WITH EQUAL OR BETTER ITEM. ANY LANDSCAPE AREA DAMAGES SHALL BE REPLACED TO EQUAL OR BETTER
CONDITION.
11. ALL BEDDING AND BACKFILL SHALL BE PER DETAIL SHOWN IN THESE PLANS.
12. WORK ON ITEMS NOT SPECIFICALLY CALLED OUT IN BID DOCUMENT SHALL BE INCIDENTAL TO OVERALL PROJECT
COST.
13. WORKING AREAS SHALL BE ONLY THOSE AREAS DESIGNATED BY OWNER. ANY ACTIVITY BEYOND THESE PREMISES IS
STRICTLY PROHIBITED. INGRESS AND EGRESS OF MEN AND EQUIPMENT SHALL BE THROUGH APPROVED EASEMENTS
ONLY. CONTRACTOR TO COORDINATE WITH OWNER PRIOR TO CONSTRUCTION.
14. ALL REMOVAL OF ASPHALT ROADWAYS, AS DESIGNATED BY CITY TO BE OPEN CUT SHALL BE PROPERLY SAW CUT,
AND THEN BACKFILLED PER DETAIL SHOWN.
15. CONTRACTOR SHALL BE RESPONSIBLE FOR HAULING AND DISPOSAL OF THE EXISTING AND PROPOSED EXCAVATED
MATERIAL AT THIS OWN EXPENSE UNLESS OTHERWISE DIRECTED BY THE ENGINEER TO DELIVER TO A LOCATION
WITHIN ONE MILE RADIUS FREE OF CHARGE.
16. NO EXCAVATION AREA SHALL BE LEFT OPEN DURING NON -WORKING HOURS. ALL EXCAVATION OVER 2 FEET IN
DEPTH SHALL BE PROTECTED BY BRIGHT ORANGE SAFETY FENCING.
- 17. CONTRACTOR SHALL NOTIFY THE CITY OF LA PORTE AT 281-471-9650 TWENTY-FOUR (24) HOURS PRIOR TO ANY
UTILITY COORDINATION REQUIRED.
18. ALL SIGNS REMOVED SHALL BE RESET UNLESS OTHERWISE NOTED. ANY SIGN DAMAGED SHALL BE REPLACED WITH
NEW SIGN AT NO COST TO OWNER.
19. ALL GRASS AREAS AFFECTED BY CONTRACTORS OPERATIONS SHALL BE LEVELED TO ORIGINAL CONDITIONS &
HYDROMULCHED.

SHALL EXPOSE THE SOUTHWESTERN BELL TELEPHONE FACILITIES.
C. WHEN SOUTHWESTERN BELL TELEPHONE FACILITIES ARE EXPOSED THE CONTRACTOR
SHALL PROVIDE SUPPORT TO PREVENT DAMAGE TO THE CONDUIT DUCTS OR
CABLES. WHEN EXCAVATING NEAR TELEPHONE POLES THE CONTRACTOR SHALL BRACE THE POLE
FOR SUPPORT.
The Contractor shall be the operator as defined by TPDES General Permit. The contractor shall
comply with all TPDES General Permit requirements to. include preparation, maintenance and
implementation of the SW3P.
The Contractor shall submit all notifications and pay all associated fees required by the TPDES
General Permit. The contractor shall provide the City Inspector copies of any submitted
notifications and associated fee checks.
The Contractor's temporary control measures shall be installed and maintained throughout the
construction contract and coordinated with proposed or existing permanent pollution control SANITARY SEWER NOTE:
features to assure effective and continuous water pollution control throughout the construction REMOVE LATERALS, CUT AND PLUG, RECONNECT TO
PROPOSED SANITARY SEWER LINE OR M.H.
and post construction period. The Contractor will maintain and inspect the temporary control
measures in accordance with the TPDES General Permit requirements.

GENERAL NOTES
1. TRENCH SAFETY SYSTEM TO MEET, AS A MINIMUM, THE REQUIREMENTS
OF OSHA SAFETY AND HEALTH REGULATIONS PART 1926, SUBPART P
AND Q.
2. THE CONTRACTOR MAY ELECT TO USE AN ALTERNATE SYSTEM TO THE TIM-
BER TRENCH SHORING SHOWN IN THE TABLE. THE ALTERNATE SYSTEM, -BE
IT A TRENCH BOX, STEEL SHEET PILING, TRENCH JACKS OR A COMBINATION
OF THE ABOVE, MUST BE CERTIFIED BY THE CONTRACTOR'S REGISTERED
PROFESSIONAL ENGINEER THAT IT PROVIDES EQUAL OR GREATER PROTEC-
TION THAN THE TIMBER TRENCH SHORING SHOWN ON THIS DETAIL SHEET.
3. CONTRACTOR SHALL PERFORM DAILY TRENCH SAFETY SYSTEM INSPECTIONS
TO INSURE THAT THE SYSTEM MEETS OSHA REQUIREMENTS AND IS.APPRO-
PRIATE FOR SPECIFIC SITE CONDITIONS OF THE OPEN TRENCH. INSPEC-
TIONS ARE REQUIRED AFTER RAINSTORMS OR ANY CHANGE IN CONDITIONS
THAT MAY INCREASE THE POSSIBILITY OF A CAVE-IN OR SLIDE.
4. IN THE EVENT THAT TRENCH UPRIGHTS CAN NOT BE PLACED BELOW TRENCH
BOTTOM AS SHOWN ON DETAIL, CONTRACTOR MUST PROVIDE ENGINEER WITH
PLANS THAT DEMONSTRATE THAT THE UPRIGHTS WILL BE BRACED AND
MAINTAINED IN A VERTICAL POSITION.
5. WHERE THE TRENCH SAFETY SYSTEM CROSSES A UTILITY, THE UTILITY
LINE MUST BE ADEQUATELY SUPPORTED TO PREVENT ANY DAMAGE. IN
ADDITION, IN TRENCHES WITH CLOSED SHEETING, THE UPRIGHTS MUST
SURROUND THE UTILITY. OPEN TRENCH SHEETING SPACING MUST BE
ADJUSTED AS TO NOT EXCEED THE MAXIMUM ALLOWED SPACING.
(NO SEPARATE PAY)
6. TRENCH SHALL BE DRAINED AS REQUIRED SO WORK MAY BE ACCOMPLISHED
SAFELY AND EFFICIENTLY. IF NECESSARY, INSTALL DEWATERING SYSTEM
TO PROVIDE A DRY TRENCH BOTTOM. DELIVER DISCHARGE FROM SYSTEM TO
NATURAL DRAINAGE CHANNEL OR TO STORM DRAINS.
7. IN TRENCHES FOUR FOOT DEEP OR MORE, CONTRACTOR TO PROVIDE ADE-
QUATE MEANS OF TRENCH EXIT SUCH AS LADDER OR STEPS AND THEY
MUST BE LOCATED SO AS TO REQUIRE NO MORE THAN 25 FEET OF
LATERAL TRAVEL.
8. MEASURE "PERMANENT TIMBER TRENCH SHORING" BY LINEAR FOOT OF
TRENCH PROTECTED. PAYMENT IS BY STATED SAME LINEAR FEET OF TRENCH
PROTECTED EARTH CONDITIONS NOTED IN THE PROPOSAL (BID).
9. STRINGERS AND BRACES TO BE SECURELY FASTENED.
10. TRENCH SHORING SYSTEM HAS BEEN DESIGNED WITH NO ALLOWANCE
FOR LIVE LOAD SURCHARGE. IF LIVE LOAD SURCHARGE IS ANTICIPATED
IT SHALL NOT BE PLACED WITHIN 2 FEET OF EDGE OF TRENCH AND CONTRAC-
TOR'S REGISTERED PROFESSIONAL ENGINEER SHALL DESIGN TRENCH SHOR-
ING SYSTEM TO ACCOMMODATE THE ANTICIPATED LIVE LOAD SURCHARGE.
11. TIMBER SIZES ARE BASED ON STRESSES OF 1650 PSI FOR EXTREME FIBER
IN BENDING AND 1450 PSI FOR COMPRESSION PARALLEL WITH GRAIN.
12. EARTH TRENCH CONDITIONS FOR THIS PROJECT ARE ANTICIPATED TO BE
AS SHOWN IN THE GEOTECHNICAL REPORT AVAILABLE FROM ENGINEER.
13. TIMBER TRENCH SHORING TABLE SHOWN ON THIS DEATIL SHEET IS GOOD
FOR PIPES WITH O.D. OF UP TO 48 INCHES. THE DESIGN ENGINEER SHALL
PROVIDE SEPARATE DESIGN FOR PIPE WITH O.D. LARGER THAN 48 INCHES.
16. SLOPES FOR EXCAVATIONS SHALL BE AS FOLLOWS:
A) SOLID ROCK, SHALE OR CEMENTED SAND & GRAVELS = 90°
B) COMPACTED ANGULAR GRAVELS =1/2: 1
C) RECOMMENDED SLOPE FOR AVERAGE SOLIDS =1 : 1
