STREET & BRIDGE NOTES: 1. EXISTING PAVEMENT CURBS, SIDEWALKS AND DRIVEWAYS DAMAGED OR REMOVED DURING CONSTRUCTION SHALL BE REPLACED TO THE CITY OF LA PORTE STANDARDS. 2. CONDITION OF THE ROAD AND/OR RIGHT-OF-WAY, UPON COMPLETION OF JOB SHALL BE AS GOOD AS OR BETTER THAN PRIOR TO STARTING WORK. PAVEMENT NOTES: 1. THE SUBGRADE SHALL BE SCARIFIED TO A DEPTH OF 6" AND LIME STABILIZED WITH SUFFICIENT LIME TO REDUCE THE PLASTICITY INDEX TO 15 OR LESS. THE LIME AND SOIL SHALL BE THOROUGHLY MIXED, CURED AND COMPACTED TO AT LEAST 95 PERCENT OF MAXIMUM STANDARD PROCTOR DENSITY. 2. LIME STABILIZATION SHALL BE DONE IN ACCORDANCE WITH THE SITEWORK SPECIFICATIONS & IN ACCORDANCE WITH THE GUIDELINES AS OUTLINED BY THE GEOTECHNICAL ENGINEER. 3. THE FLEXIBLE BASE COURSE SHALL BE CRUSHED STONE CONFORMING TO THE STANDARD SPECIFICATIONS, AND SHALL BE PLACED IN LOOSE LIFTS NOT TO EXCEED 4 INCHES IN THICKNESS AND COMPACTED TO A MINIMUM OF 95 PERCENT OF THE MAXIMUM DENSITY AS DEFINED BY ASTM D-1557 (MODIFIED PROCTOR). 4. THE HOT MIX ASPHALTIC CONCRETE SHALL BE DESIGNED AND INSTALLED TO CONFORM TO THE REQUIREMENTS OF THE STANDARD SPECIFICATIONS. A CURRENT (LESS THAN ONE YEAR OLD) MIX DESIGN SHALL BE FURNISHED BY THE PAVING CONTRACTOR TO THE TESTING FIRM FOR EACH TYPE ASPHALTIC CONCRETE TO BE USED ON THIS PROJECT. 5. FOR ASPHALTIC CONCRETE PAVEMENT SECTIONS IN EXCESS OF TWO INCHES IN THICKNESS, THE REQUIRED ADDITIONAL COURSE THICKNESS MAY CONSIST OF TYPE "D" HOT MIX ASPHALTIC CONCRETE PRODUCED & PLACED IN ACCORDANCE WITH TXDOT ITEM 340. 6. CONTRACTOR IS TO ENSURE A MINIMUM 4 INCH CURB FROM TOP OF FINISHED ASPHALT TO TOP OF EXISTING CONCRETE CURB SURROUNDING BUILDING UPON COMPLETION OF PROJECT. GENERAL NOTES: 1. THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE REMOVAL AND LEGAL DISPOSAL OF ALL WASTE MATERIAL GENERATED DURING CONSTRUCTION. WASTE MATERIAL MUST BE REMOVED FROM THE WORK SITE AND DISPOSED OF IN SUCH A MANNER AS TO NOT DAMAGE THE OWNER OR OTHER PERSONS. (NO SEPARATE PAY) 2. THE CONTRACTOR SHALL COORDINATE HIS CONSTRUCTION SCHEDULE WITH THE ENGINEER PRIOR TO COMMENCING WORK. 3. THE CONTRACTOR SHALL ADJUST EXISTING UTILITY STRUCTURES TO BE FLUSH WITH PROPOSED FINISHED GRADE. WITHIN THE LIMITS OF CONSTRUCTION. THIS SHALL INCLUDE BUT NOT LIMITED TO: MANHOLE RIMS AND COVERS, WATER LINE VALVES AND GAS VALVES. EXISTING COVERS COVERED WITH ASPHALT CANNOT BE REUSED AND MUST NOT BE REPLACED. 4. ANY DAMAGE TO ANY OF THE EXISTING UTILITIES MUST BE REPAIRED IMMEDIATELY. THE CONTRACTOR MUST NOTIFY THE APPROPRIATE UTILITY OWNER, WHO WILL MAKE THE REPAIRS AT THE CONTRACTOR'S EXPENSE. 5. CONSTRUCTION STAKING WILL BE PROVIDED BY THE CONTRACTOR. 6. THE CONTRACTOR SHALL OBTAIN ALL CONSTRUCTION PERMITS REQUIRED BY THE CITY OF LA PORTE. 7. THE CONTRACTOR MUST KEEP ALL EXISTING STREETS CLEAN OF CONSTRUCTION RELATED DEBRIS AT ALL TIMES. 8. THE FINISHING PAVEMENT SURFACE SHALL NOT DEVIATE FROM THE ESTABLISHED GRADE AND CROSS-SECTION MORE THAN 1 8 INCH IN TEN FEET WHEN TESTED WITH A STRAIGHT EDGE PLACED PARALLEL TO THE CENTERLINE AND NOT MORE THAN 1 4 INCH IN TEN FEET AT RIGHT ANGLES TO THE CENTERLINE.
