Contractor shall be aware that utilities exist or may exist in the construction area which: a. Have not been field verified (NFV) b. Have not been located on the drawings c. May not be shown accurately on the drawings d. May exist in the construction area and not shown on the drawings It is the contractors responsibility to verify the location and depth of all existing utilities prior to excavation. 2. Contractor shall contact all pertinent utility companies 72 hours (minimum) prior to excavation in area. The Houston Area Utility Coordinating Committee may be contacted for certain utilities at (713) 223-4567 and the Railroad Commission of Texas contacted for verification of Natural Gas Lines and other Utility Lines in the Construction Area at 8001 545-6005. 3. Contractor shall veri y a 1 dimensions and conditions in the field before commencing work. It shall be the Contractor's responsibility to report any and all discrepancies to the Owner and the Engineer in a timely manner. 4. The Contractor shall maintain access to Residential and Commercial properties adjacent to work areas at all times. 5. The Contractor shall conduct his operations in a manner such that trucks and other vehicles do not create a dirt nuisance or safety hazard in any streets, public or private. Clean up of streets shall be done daily. 6. The contractor shall regrade all ditches and replace all culverts and driveways to match existing conditions, unless otherwise shown. Adequate drainage and driveway ingress and egress shall be main- tained during construction. 7. When construction occurs in City Right -of -Ways, and Easements adjacent to Residential yard areas and private property, Contractor shall take care to minimize construction damage to affected yard areas and private property. 8. Contractor shall replace damaged area including but not limited to fences, landscaping to original condition and to the City's satisfaction. 9. Contractor shall be responsible for contacting the appropriate gas company to relocate any gas lines exposed due to excavation and in need of relocation. 10. No open excavations shall be left open overnight. All excavations which cannot be backfilled overnight shall be covered, as a minimum, with steel plating when in paved areas; 3/4" plywood, wood planking with OSHA orange plastic expanded mesh barrier around perimeter in unpaved areas, or as approved by the City of La Porte. 11. Existing pavements, curbs, sidewalks and driveways damaged or removed during construction by the Contractor shall be replaced in kind by the Contractor at his expense. 12. Existing utility lines shall remain in place, unless otherwise shown or directed by the Engineer. 13. Contractor shall avoid interruption of utility service to any Resident or Business. Any such interruption shall be closely co-ordinated with the City. Contractor shall provide notices of service interruption to the residents or Business in the manner described in the specifications. 14. Contractor shall not operate City water valves. Contractor shall contact City of La Porte for any valve operation necessary for this project. Contractor shall verify that proper notices have been provided to affected residents or business prior to the closing of any valves. 15. Contractor shall not use Residents' water. City will provide water meter and water at no charge to the contractor. 16. Contractor shall Provide one foot (1') minimum clearance between gas lines and all other utilities (New or Existing). 17. On all pavements to be removed, including concrete driveways and sidewalks, the pavement shall be sawcut prior to removal. 18. All excavated areas shall be barricaded at all times to prevent motorists from inadvertently driving into bore pits, open cuts, and - trenches. All excavation must also be cordoned off with 4-foot high orange polyethylene fence with mesh size 2.50 inches by 0.80 inch, Tenox Beacon or approved equal, installed per manufacturers recommendations and acceptable to the City of Lo Porte. The fence shall be supported with enough intermediate support to avoid excessive sagging. The fence may be tied to traffic barricades where excavation extends beyond or is located entirely behind the existing back -of -curb or edge of pavement. Adequate barricades with flashing yellow lights shall also be provided to protect pedestrian traffic. 19. Contractor shall take necessary measures to insure that light standards and/or poles are adequately braced and supported when excavation or construction activity occurs in their vicinity. The cost associated with such efforts shall be insidental to the project. 20. Contractor shall adequately protect existing structures, utilities, trees, shrubs and other permanent objects. All costs associated with repairs or replacement of damage due to Contractors performance of work will be paid by the Contractor at no additional cost to the project. 21. Trees within the Street Right -of -Way shall not be removed nor disturbed. Where tree roots must be cut, obtain City's concurrence and follow the repair methods described in the Specifications. 22. Any area of grass which is disturbed or dug up during the construction shall be replaced with St. Augustine sod or grass which matches the gross removed. 23. All manholes and other utility structures in the construction area shall be adjusted to finished grade. Rim elevations for manholes shall be set flush with the finished grade for all manholes located in pavement. Rim elevations shall be 3 inches above natural ground for all manholes located in other areas unless otherwise shown. For manholes outside of pavement, sloped fill shall be added for storm water drainage away from manhole rim. 24. Existing pavements, curbs, sidewalks and driveways damaged during construction by the Contractor shall be replaced by the Contractor at his expense. 25. Texas Low Article 1436c, prohibits all activities in which persons or equipment may come within six feet of energized overhead power lines, and Federal Regulations, Title 29, Part 1910.180(i) and Part 1926.550(a)(15) require a minimum clearance of ten feet from these facilities. The above laws carry both criminal and civil liabilities, with contractors and owners being legally responsible for the safety of workers under these laws. If you or your company must work near overhead power lines, call (71 )-207-7777 for the lines to be de -energized and/or moved at your expense. 26. Contractor to coordinate with the City as necessary and when required regarding "Special Events" that may be affected by construction. These events may require consideration by the Contractor (construction schedule or sequence, cleanup, street and sidewalk cleaning, removal and disposal of debris etc.) The Contractor shall perform this work at no additional cost to the project. 27. The preparation of these plans reflect information provided by others on the approximate location and existence of existing utilities and adjacent physical features; however, they do not imply or affirm that all utilities or physical features are shown. Contractor is responsible for notification of the Owner immediately upon encountering unforeseen conflicts. NOTE: The locations of Southwestern Bell Telephone Company facilities are shown in an approximate way only. The contractor shall determine the exact location before commencing work. He agrees to be fully responsible for any and all damage which might be occasioned by his failure to exactly locate and preserve these underground utilities. The contractor shall contact the Utility Coordinating Committee at (713) 223-4567 or 1-800-669-8344 a minimum of 48 hours prior to construction to have underground lines field located. When excavating within eighteen (18 in) of the indicated location of Southwestern Bell Telephone facilities, all excavations must be accomplished using non -mechanized excavation procedures. When Southwestern Bell Telephone facilities are exposed, the contractor should provide support to prevent damage to the conduit ducts or cables. When excavating near telephone poles the contractor shall brace the pole for support. WARNING: HL&P FACILITIES 'cant Energy HLatF- Overhead lines may exist on the property. We have not attempted to mark those lines since they are clearly visible, but the contractor should locate them prior to beginning any construction. Texas law, section 752, Health & Safety Code, forbids all activates in which persons or things MAY come within six (6) feet of live overhead high voltage lines. Parties responsible for the work, including contractors, are legally responsible for safety of construction workers under this law. This law carries both criminal and civil liability. To arrange for lines to be turned off or removed, call RELIANT ENERGY HL&P at 713-207-7777. CAUTION: UNDERGROUND GAS FACILITIES Reliant Energy Entex- Locations of Reliant Energy Entex main lines (to include Unit Gas Transmission and/or Industrial Gas Supply Corporation where applicable) are shown in an approximate location only. Service lines are usually not shown. The contractor shall contact the Utility Coordinating Committee at (713) 223-4567 or 1-800-669-8344 a minimum of 48 hours prior to construction to have main and service lines field located. * When Reliant Energy Entex pipe line markings are not visible, call (713) 967-8037 (7:00 a.m. to 4:30 p.m.) for stautus of line location request before excavation begins. *When excavating within eighteen inches (18") of the indicated location of Reliant Energy Entex facilities, all excavation must be accomplished using non -mechanized excavation procedures. * When Reliant Energy Entex facilities are exposed, sufficient support must be provided to the facilities to prevent excessive stress on the piping. The Contractor shall coordinate with United States Army Corps of Engineers (U.S.A.C.O.E.) for construction of the proposed sanitary sewer by—pass across the drainage channel. The Contractor shall be aware that the City of La Porte has notified U.S.A.C.O.E of the proposed construction in the said areas.
