<br />e <br /> <br />EXHIBIT A ,- <br />RETIREE MEDICAL COVERA <br /> <br />ELIGIBILITY <br /> <br />Medical Plan shall be changed as follows: <br /> <br /> <br /> <br /> <br /> <br />PREMIUMS <br /> <br />The City of La Porte utilizes a self funded medical plan so there are no formal premiums <br />paid for health insurance. Each year, during the budget process, an amount is <br />determined for each employee to cover claims and administration of the medical fund. <br />The current amount is $4,500 per employee, <br /> <br />Costs to the retiree shall be based on a three tier plan: <br /> <br />~. <br />l'l'R.Jim <br /> <br />The first tier shall be based on a point system consisting of a combination of age and <br />continuous years of service with the City of La Porte. Qualification for this coverage <br />requires age and years of service to total at least 80. <br /> <br />The City will pay 100% of coverage for eligible retirees. The retiree shall be responsible <br />for paying for dependent coverage at the active employee rate. <br /> <br />Example: <br /> <br />New hire at 20 years of age - Qualifies at age 50 with 30 years of service, <br />New hire at 40 years of age - Qualifies at age 60 with 20 years of service. <br /> <br />Retiree Medical Coverage <br />Page 2 <br />
