<br />e <br /> <br />e <br /> <br />1.0 PROJECT SUMMARY <br />Bayshore Comrmmity Development, Inc., (BCDI), a not for profit, COITm.l- <br />ni ty based-organization proposes to implement a Transportation Services Pro- <br />ject to the City. of La Porte for $24,698. Services will be geared to trans- <br />{X)rting primarily elderly, handicapped, and low-incorre residents to medical <br />facilities, social service agencies (entitlement programs) pharmaceutical <br />enti ties, one-a-week shopping and selected enrichrrent and cultural acti vi- <br />ties. Trans{X)rtation services will be provided to a rronthly ridership of <br />800 persons on a scheduled, subscribed demand res{X)nse basis, picking up <br />eligible residentsfram their horres, tran.s{X):r:ting them to scheduled appoint- <br />ments and/or activities and retunring them to their residences. Residents <br />must call in a 24-hour lead time basis. <br /> <br />This Project represents a continuation of services previously funded <br /> <br />through a one-time Special Ini tiati ve Grant awarded by the Comrmmi ty <br /> <br />Services Administration. BCDI. is also exploring the expansion of transpor- <br /> <br />tation service with other {X)litical subdivisions in the Bayshore area. <br />
