<br />e <br /> <br />e <br /> <br />ANIMAL CONTROL REPORT <br />Page 4 of 9 <br /> <br />To register an animar, the pet owner must bring proof of <br />vaccination from a licensed veterinarian to City Hall. Once <br />there, the Revenue Collection Clerk receives the registration fee, <br />and logs-in the entry by completing a receipt with the following <br />information: <br /> <br />a. owner's residential and telephone number; <br /> <br />b. animal tag number; <br /> <br />c. amount of fee received from owner; <br /> <br />d. name of animal (dog or cat); <br /> <br />e. type of animal; and <br /> <br />f. coloring of animal. <br /> <br />The pet owne r is then given a rece ipt and a metal 1 icense tag. <br />This tag is to be placed on the animal's collar for identification <br />purposes. To insure that the animal is registered annually, the <br />City changes the design and color of the metal license tag. The <br />City's annual registration fee is $5.00 if registered during <br />January to March, and $7.00 during April to December. <br /> <br />Information inscribed on the metal tag includes the year issued, <br />the City's name, and the 1 icense number. Howeve r, should the <br />animal not have its rabies vaccination, the owner can purchase a <br />vacc inat ion for a $10 fee from the City's Revenue Collect ion <br />Clerk. This form (see attachment IV) can be redeemed for a rabies <br />vaccination with the City's veterinarian. The pet owner then <br />receives a separate metal license tag to indicate that the animal <br />has been given a rabies vaccination. Information inscribed on the <br />front side of the license tag issued by the veterinarian include: <br />the year issued, the clinic's name, and the license number. The <br />animal's name and the owner's phone number are inscribed on the <br />back side of the license tag. <br /> <br />Presently, there are two (2) veterinary clinics in La Porte, and <br /> <br />both distribute metal <br />information inscribed <br />similar; however, one <br />side with the animal's <br /> <br />license tags to vaccinated animals. The <br />on the front side of the metal tags is <br />of the clinics did not inscribe the back <br />name and owner's phone number. <br />
