<br />City of La Porte-Public Works <br />PSI Proposal No.: 0204-30915 <br />October 1,2010 <br />Page 2 of 10 <br /> <br />PSI proposes to provide experienced qualified technical personnel to perform services <br />in general accordance with the project plans and speCifications. PSI will perform these <br />services as requested by the client. Michael J. Whitecotton, PE will be the responsible <br />registered Engineer for your project. <br /> <br />Scope of Services: <br /> <br />PSI understands the project will consist of the installation of 48" RCP and varying sizes <br />of ReB Storm Lines, manholes, inlet boxes, concrete pavement, concrete curbs, <br />concrete handicap ramps, and replacement concrete drive entrances. <br /> <br />Based on this information, the duration of the project, the plans, City of LaPorte <br />Specifications, Harris County Specifications, and our experience with similar projects, <br />PSI proposes the following scope of services for your project. <br /> <br />1. Earthwork - The earthwork scope includes the inspections and testing of the <br />installation of the RCS placement, RCP Pipe placement, Manhole installation, <br />inlet installation, swale compaction testing, and lime stabilization of the paving <br />subgrade. PSI will perform the following field inspections and testing for the items <br />described above, as per the project specifications. <br /> <br />a) Sample subgrade, select fill, trench backfill, cement stabilized sand, and <br />lime stabilized paving subgrade. Prepare and test the samples for <br />classification by the Atterberg Limits, and moisture density relationship. <br /> <br />b} Perform field density tests using the nuclear method to determine the <br />moisture content and percent compaction of the subgrade, cement <br />stabilized sand, fill, and backfill. <br /> <br />c) Perform Lime Slurry placement inspections, gradations, and compaction <br />testing. <br /> <br />2. Concrete- Inspections and testing of the concrete is anticipated to be performed on <br />sidewalks, curb ramp, paving, concrete swale, and concrete headwall. The <br />concrete inspections will include the following, per the project specifications and <br />Harris County Specification Items. <br /> <br />a) Inspecting the reinforcing steel prior to the placement of the concrete. <br /> <br />b) Sample the fresh concrete for each mix, perform required tests, including <br />slump, air content, concrete temperature, and cast test specimens <br />during placements. <br /> <br />c) Perform compressive tests of concrete test cylinders cast in the field. <br />l"'~;l <br />~..,. . <br />
