SAFETY STANDARDS AND ACCIDENT PREVENTION <br />With respect to all work performed under this contract, the Contractor shall: <br />(1) Comply with the safety standards provisions of applicable laws, building and <br />construction codes and the "Manual of Accident Prevention in <br />Construction" published by the Associated General Contractors of <br />America, and the requirements of the Occupational Safety and Health <br />Act of 1970 (Public Law 91-596). <br />(2) Exercise every precaution at all times for the prevention of accidents and <br />the protection of persons (including employees) and property. <br />9. CONFLICT ON INTEREST <br />Chapter 176 of the Texas Local Government Code requires that any person, who enters or <br />seeks to enter into a contract for the sale or purchase of property, goods or services with a <br />local government entity and who has an employment or other business relationship with a <br />local government officer or a family member of the officer, as described by Texas Local <br />Government Code Section 176.006, shall file a completed conflict of interest questionnaire <br />with the City Secretary within 7 business days after the latter of: 1) the date the person <br />begins discussions or negotiations to enter into a contract, including submission of a bid or <br />proposal, or 2) the date the person becomes aware of facts that require the statement to be <br />filed. The Conflict of Interest Questionnaire (Form CIQ) is available from the City of La Porte <br />website at www.la ortetx. ov or from the Texas Ethics Commission at <br />www. ethics. state. us. Completed conflict of interest forms may be mailed or delivered to the <br />office of City Secretary, 604 W. Fairmont Pkwy,. La Porte, TX 77571. Please consult your <br />own legal advisor if you have questions regarding the statute of this form. <br />10. SUPPLEMENTAL INFORMATION <br />A. All prices to be F.O.B., Destination, La Porte, TX 77571. Additional freight charges will <br />not be considered. <br />B. The City of La Porte is exempt from all taxes in the State of Texas, including sales tax. <br />A tax-exempt form will be provided upon request. <br />C. Brand names in specifications are strongly preferred; alternates must be approved <br />by city personnel prior to bid opening. <br />D. All exceptions to the specifications and/or brand names must be so stated on the <br />bid. <br />
