PROPOSAL DETAILS <br />Interested firms shall submit an original and three (3) copies of their proposal. <br />Statement of Qualifications <br />• A detailed description of experience with service requested, including the <br />number of years and specific areas of knowledge and experience; <br />• Primary contractor's experience and background, and/or experience and <br />background of all principals of the firm, particularly experience within the <br />Houston metropolitan area; <br />• Current work load of primary contractor or firm; <br />• Disclose any known conflicts of interest by principals in the firm that <br />could result in your firm; and <br />• List of past/current municipal clients (including primary contact and phone <br />number) for which similar work was completed. <br />Proposal of Charges and Fees <br />Proposers must utilize the attached form to provide information regarding specific <br />charges and fees for services. <br />• Charges and fees for general performance of the Scope of Services, as <br />outlined above; <br />• The firm's policy regarding charges for travel including charges for travel <br />time to and from meetings; and <br />• The firm's policy regarding charges for reimbursable expenses. <br />ATTACHMENTS: <br />Please attach the following documents to the original proposal and all copies. <br />• All City required forms <br />• Statement of Qualifications <br />• Proposal Form (included in the RFP as Appendix A) <br />• References (included in the RFP as Appendix B) <br />
