• <br />0 <br />Page 6 of 12 <br />SUBJECT: Injury Suffered While on Duty <br />1. The City Commission has elected to take out an insurance <br />policy for the City with a local firm. <br />2. An employee injured while in the performance of his <br />assigned duties for the City, no matter how slight <br />the injury, shall immediately report to his supervisor <br />and receive first aid and be taken to a doctor or a <br />Hospital. <br />3. NOTE TO DEPARTMENT HEADS: Fill our accident report, take <br />statements of fellow workers who witnessed the accident <br />and see that this information is attached to the attending <br />physician's report and turned into the office. <br />4. Time and amount an employee is paid while off duty, due to <br />injury, is figured according to rates per our policy. <br />• EXAMPLE: Workmen's Compensation will take precedence and <br />pay first when an employee is injured on the job. <br />At this writing, 5/25/76, Workmen's Compensation pays <br />$63.00 per week, starting on the 8th day. <br />An employee's sick leave pay will be used to <br />supplement the Workmen's Compensation, but in no <br />event is the employee to receive more than his <br />regular salary. Employee will not forfeit sick <br />leave time when injured on the job. <br />EXAMPLE: Employee's salary is $100.00 per week; he has <br />sixty days accumulated sick leave and he gets <br />hurt on the job: <br />He will be paid $63.00 a week from Workmen's Compensation and <br />$37.00 a week sick leave pay making a total of $100.00 <br />60 days sick leave = 12 wks. or $1200.00 <br />$1200.00 ; $37.00 = 32.4 wks. that sick <br />leave will supplement Workmen's Compensation. <br />After this time, if employee is unable to <br />return to work, he will receive $63.00 a week <br />Workmen's Compensation for the period provided <br />by law. <br />
