Box 211 <br />La Porte,-Texas 77571 <br />Dear Mr. Carpenter: <br />On October 1, 1975, the City of La Porte entered into an agree- <br />ment with La Porte Ambulance Service, Inc., for a two-year term, <br />for La Porte Ambulance Service, Inc., to provide ambulance sere- <br />. ice within the City of La Porte. This agreement was amended by <br />letter agreement of October 27, 1975, relating to the lease by <br />the City to your firm, of one CPR. machine, and. three hospital <br />• encoders. <br />The City of La Porte proposes that this basic agreement, as <br />amended, be extended for a five-year period commencing October 1, <br />1977, and terminating September 30, 1982, on the same terms and <br />conditions of the basic agreement,, as amended, except as amended <br />as follows: <br />(1) That the City of La Porte (hereinafter "City") agrees <br />and promises to pay La Porte Ambulance Service, Inc. <br />(hereinafter called.."Contractor"), Six Thousand Seven <br />Hundred Fifty-five Dollars ($6,755.00) per month for <br />such services. <br />(2) Paragraph I(d) shall be amended, to provide that <br />Contractor shall not be required to provide an attendant, <br />in addition to the driver, on invalid coach service. <br />(3) Paragraph IV shall be amended, to the effect that patients <br />being picked up shall have the right to designate any <br />hospital in Harris or Galveston Counties, which is within <br />a fifty-mile radius of the City of La Porte. <br />• <br />
