• • <br />''~.a <br /> <br />Mr. Jerry Carpenter, President Page 2 <br />• La Porte Ambulance Service, Inc. 10/1/77 <br />(4) Paragraph V shall be amended to read as follows: <br />"V. <br />CHARGES PERMITTED CONTRACTOR <br />"Contractor shall be permitted. to charge not more than the <br />following, to-wit: <br />(1) Ambulance Service and Invalid Coach Service: <br />(a) $45.00 base fee for transporting one <br />person to a hospital in Baytown, <br />Pasadena, or Clear Lake City. <br />(b) $65.00 base fee for transporting one <br />person to_a hospital in Houston <br />or Galveston. <br />• (c) $5.00/15 min. charge, if any, for waiting in <br />excess of fifteen minutes. <br />(d) $20.00 charge, if any, for unfounded <br />requests for service. <br />(e) $25.00 maximum charge for transporting <br />one person to a doctor's office <br />in La Porte, from location within <br />the limits of the City of La Porte. <br />(f) Method of dividing cost among patients if more <br />than one is carried in the same ambulance simul- <br />taneously to one hospital: <br />One and one-half times the regular rate, divided <br />z by the number of patients transported; each <br />patient to pay equal amount. <br />"The collection of such fees shall be the sole responsibility <br />of the Contractor." <br /> <br />
