(BHI) is pleased to respond to your request for a proposal to <br />extend EAP services to the City of La Porte. We enclose our proposal in which we <br />present our philosophy of triage therapy, a review of how our services are provided, <br />and a price quote for a 1-8 session EAP for your regular, full-time employees and their <br />dependents, In addition, we include a copy of our EAP Clinical Manual. A third <br />enclosure ("Competency and Services Overview" booklet) includes bios of our key <br />staff, a listing of facilities and individual providers of service, and general information <br />about our company. <br /> <br />Behavioral Health, Inc. is a not-for-profit mental health services company that has <br />served the needs of the people of this region for seven years. We are part of the <br />General Health System, a $190M company which has been in existence since 1904. <br />We enclose a copy of our most recent financial report prepared by Ernst & Young. <br /> <br />Our statement of philosophy is presented in the initial part of the proposal. We wish to <br />respond specifically to several of the questions covered in your May 18th Request for <br />Proposal letter. <br /> <br />. The account managers will be Dr. Larry V. Stockman assisted by Edwin Edghill, with <br />primary responsibility being Dr. Stockman. <br /> <br />. Local and toll-free telephone service (24 hour service) is provided. The availability <br />of a local telephone number as well as an 800 number will assist traveling <br />employees or their dependents who may be attending school outside of the local <br />area. That number is 1-800-422-6963, and both numbers will be printed on all City <br />of La Porte business cards and EAP tri-folds. <br />
