<br />. <br /> <br />(5) The chief EMS officer must be notified within twenty-four (24) hours of any <br />cancellation of insurance coverage. <br /> <br />Section 7 Application for attendant/driver's license <br /> <br />(a) Applications for attendant/driver's licenses hereunder shall be made upon such form <br />as may be prepared by the EMS division and shall contain the following information <br />which is a requirement for the issuance of a license. <br />(1) The applicant's full name, current address, how long the applicant has been at <br />current address, previous address, how long at that residence, home telephone <br />number, and social security number. <br />(2) The applicant's age, date of birth, height, weight, place of birth, hair and eye <br />color. <br />(3) The applicant's Texas driver's license number, restrictions, and expiration date. <br />(4) The applicant's present EMS certification level, registration number, and <br />expiration date. <br />(5) Two (2) recent photographs of passport size or equivalent. <br />(6) A report from a duly licensed physician of the State of Texas ofa recent physical <br />examination which attests that the person has no physical defects which would <br />impair his or her ability to perform as an ambulance attendant. <br />(7) Such other information as the chief EMS officer shall deem necessary to a fair <br />determination of compliance with this chapter. <br /> <br />. <br /> <br />(b) The chief EMS officer shall within fifteen (15) working days of receipt ofa <br />completed application for an attendant's license investigate the application and issue <br />the license or notify the applicant and the applicant's employer that the application is <br />denied. <br /> <br />Section 8 Standards for attendant/driver's license <br /> <br />(a) All attendants and drivers must be able to read, write, and speak the English <br />language. <br />(b) All drivers must possess a current, valid Texas driver's license. No conditions other <br />than Restriction (corrective lenses) will be approved. The license must be Class A, <br />B, or C. <br />(c) All attendants and drivers must hold current certification from the Texas Department <br />of Health as an Emergency Medical Technician or higher. <br />(d) No attendant or driver shall have a fmal conviction for felony theft, robbery, <br />weapons violations, state or federal controlled substances acts, rape, sexual abuse, <br />indecency with a child, or abuse of a corpse. <br />(e) No driver shall have been convicted of three (3) or more moving violations in the <br />preceding two (2) year period, nor any two year period thereafter. <br />(f) All drivers shall be able to pass the vision test as prescribed by the Texas Department <br />of Public Safety. <br /> <br />. <br />
