Any employee may include <br />his family in this group insurance program by agreeing to pay <br />their portion of the premium. <br />2. If an employee knows that he or a member of his family <br />has to be admitted to the hospital, he should notify his Supervisor <br />or Department Head before admittance. However, should it be an <br />emergency, notice should be given as soon as practical. <br />3. Notice to Department Heads <br />When employee tells you of need to go to the hospital, <br />please nitify Insurance Clerk so that she will know what <br />to tell the hospital when they call. <br />• <br />• <br />
