• <br />5. Department heads are responsible for reporting sick leave <br />time on the employee's time card. <br />• 6. If a permanent full time employee terminates after com- <br />pleting one (1) year of service, the employee shall re- <br />ceive, at separation, payment for accumulated but unused <br />sick leave hours according to the following formula: <br />Sick leave hours x Annual salary = payment <br />2080 <br />Under no~circumstances, however, shall a terminating <br />employee receive payment for more than four hundred eighty <br />(4$0) hours of sick leave compensation payment, regard- <br />less of the number of hours accumulated. In addition, <br />this section does not apply to employees being terminated <br />for just cause. <br />Section 4. Military Leave <br />Military reservists who must attend required summer training <br />or other military activity are eligible for military leave of <br />absence for a period not to exceed two (2) weeks. The City will <br />grant the employee full pay for one week of such training per <br />calendar year. Any additional time required shall be taken without <br />pay or charged to accrued vacation at the employee's discretion. <br />• Employees requiring the use of authorized military leave of <br />absence shall furnish their department head and the personnel <br />coordinator with copies of military orders or other appropriate <br />certification to document the necessity for leave. <br />Section 5. Emergency Leave <br />1. In case of sudden and/or serious illness or injury to a <br />member of an employee's immediate family which requires <br />the employee's personal attention and care, he may be <br />allowed absence with pay for a period not exceeding <br />three (3) working days. <br />2. Emergency leave is confined to three (3) working days <br />for no more than a total of two (2) times in any one (1) <br />year. <br />3. Immediate family shall be defined as wife, husband, child, <br />brother, sister, parents, grandparents, mother-in-law or <br />,father-in-law, or foster parents, but not both parents <br />and foster parents. <br />4. In case of death to member of an employee's immediate <br />• family an employee shall be granted leave with pay oc- <br />curring in the first three working days immediately <br />following the date of death. <br />18 <br />
